E-One Incorporated (E-One
) is recalling certain 2019-2021 Cyclone 2, 2020 Commercial, Quest 2, 2020-2021 Typhoon, 2021 Cyclone N, and 2020-2022 Typhoon N fire trucks. The neutral screw connection within the QO Plug-On Neutral Load Center may be loose. If that connection is loose, the affected load center can overheat.
Remedy
Dealers will inspect and repair, or replace the load center, free of charge. Owner notification letters were mailed October 24, 2022. Owners may contact E-One’s customer service at 1-352-237-1122.

Dear Valued Customer,
Schneider Electric is committed to delivering high quality products to our customers. As part of our ongoing quality monitoring process, a potential issue was detected that may affect specific QO Plug-On Neutral Load Centers manufactured between February 1, 2020 and January 12, 2022 at Schneider Electric Plant 15.
The issue detected is a loose neutral screw connection within the QO Plug-On Neutral Load Center. If that connection is loose, the affected load center can overheat, posing thermal burn and fire hazards There is a potential for PROPERTY DAMAGE AND PERSONAL INJURY as a result.
Affected products were manufactured between February 1, 2020 and January 12, 2022, with date codes between 200561 and 220233. Plant 15 is the only plant impacted and all other plant codes are not affected. A complete list of affected catalog numbers, date code description, and inspection instructions is shown in Attachment 1, Product Identification.
This nonconformity has been contained at Schneider Electric manufacturing plants, distribution centers and product adaptation locations. All QO Plug-On Neutral Load Centers produced after January 12, 2022 are conforming to Schneider Electric design and manufacturing standards.
Existing inventory with date codes prior to January 13, 2022 has been validated and is marked with a green dot to indicate compliance with Schneider Electric design and manufacturing standards. Any product marked with a green dot on the carton is excluded from this notice.
This Product Recall is happening in cooperation with and has approved by the Consumer Product Safety Commission (“CPSC”).
This letter is to communicate these risks and outline actions requested to ensure the proper identification, quarantining and return of any affected product.
While the inconvenience is regrettable, safety remains the highest priority of Schneider Electric. For this reason, thank you for your prompt assistance in this matter.
What to do:
Inspect existing inventory and stop installation of the affected products immediately.
If this activity has not been completed, please do so immediately. All affected products should be removed from stock and/or shelves and quarantined in accordance with attachment 1.

Only qualified electrical individuals should remove the cover of the load center.

July 22, 2022 – REV’s senior leadership approved a voluntary recall for this issue.

I will not make excuses and I take full responsibility for the late mailing. I waited until the last week to send the letters to the printing company. The printing company has always been able to print and send the letters within a couple of days. However, this time the company’s printing machine went down and they were having problems getting the parts to repair it which caused an additional delay. Manufacturing date codes are February 1, 2020, through January 12, 2022, using date codes YYWWDS where YY = year, WW = week, D = day, and S = shift. 200561 through 220233 with Plant Code 15 are affected. The date and plant codes can be found on the interior assembly label or box label (see Figure 1). The interior assembly label is located on the top of the plastic mains base inside of the enclosure as shown in Figure 2. The box label is located on one of the enclosure interior side walls, as shown in Figure 3. Turn off all power supplying the load center. 2. Remove screws securing the load center cover (indoor load center, see Figure 4, A) or trim (outdoor load center, see Figure 4, B) and retain screws and cover for re-assembly. Remove load center cover or trim. Figure 4 – Indoor and Outdoor Load Centers (Examples) 3. Using a properly rated voltage sensing device, verify the absence of voltage. 4. Verify if manufacturing plant code is 15, that the catalog number is on the affected list (see Annex) and the date code is between 200561 and 220233 (February 1, 2020—January 12, 2022). See Figures 1–3. NOTE: If the plant code, catalog number, and date code are not inside this scope, no remediation is necessary; re-install cover (indoor) or trim (outdoor) using screws removed in step 2 and proceed to step 9. 5. Locate the plug-on neutral (PoN) connector clamping screws securing the connector to the PoN bars on the mains base of the load center. Mains bases will have two PoN connector clamping screws. These screws are silver colored, Torx head screws. See Figure 5. Check the joints of the neutral connectors for evidence of degradation due to overheating (burns or plating discoloration) or evidence of plastic base overheating (melting). Evaluation: If there is evidence of degradation on neutral connector joints, the full interior must be replaced. Identify the catalog number shown in the interior label (Figure 1) and contact Schneider Electric Technical Support: 1-888-778-2733, options 2, 1, and 4. It is important to continue with the process in step 7 to secure neutral connections until interior is replaced. 7. Loosen left bottom neutral bar screw. a. Use a T-20 screwdriver to loosen the left bottom neutral bar screw one full turn counterclockwise (see Figure 6). Figure 6 – Left Bottom Neutral Bar Screw b. Use the screwdriver to turn the left neutral bar screw alternating clockwise and counterclockwise, while inspecting for movement on the PoN connector. See Figure 7. DO NOT tighten the clamping screw, as it may damage the connector. USING THE TORQUE WRENCH tighten the left bottom neutral bar screw to 25 lb-in. (2.8 N•m) and add a mark with the black permanent marker. See Figure 8. Go to step 8. Tighten the clamping screw 1/4 turn at a time, verifying for movement after each 1/4 turn, until the connector does not move anymore. When the connector will not move anymore, apply a mark with the black permanent marker, between the connector and clamping screw. See Figure 10. USING THE TORQUE WRENCH, RE-TIGHTEN the left bottom neutral bar screw to 25 lb-in. (2.8 N•m). Add mark to the left bottom neutral bar screw with black permanent marker. See Figure 11. Figure 11 – Apply Marking d. Use the mark on the clamping screw as a reference to tighten an additional 1/4 turn clockwise (see Figure 12). Alternate: use torque wrench to torque clamping screw to 25 lb-in. (2.8 N•m). NOTICE HAZARD OF EQUIPMENT DAMAGE Do not overtighten clamping screws. After the clamping screw has made contact with the plug-on neutral bar, do not tighten the screws to more than 25 lb-in. (2.8 N•m), if using a torque wrench. Failure to follow these instructions can result in equipment damage. REPEAT steps to determine if the right clamping screw is loose and must be tightened. 8. Loosen right bottom neutral bar screw. a. Use a T-20 screwdriver to loosen the right bottom neutral bar screw one full turn counterclockwise (see Figure 13). Use the screwdriver to turn the right neutral bar screw alternating clockwise and counterclockwise, while inspecting for movement on the PoN connector. See Figure 14. (2.8 N•m) and add a mark with the black permanent marker. See Figure 15. Go to step 9. After completing inspection and/or remediation of both connectors, use the provided green dot sticker labels “Inspected PRB-236759” and place them in both locations inside the box (A) and on the back of the cover door (B). Alternate: If the green dot labels are not available, use a black permanent marker and write the legend “Inspected PRB-236759”. If the permanent marker is used during remediation, the contractor is to return at the earliest possible date to place a green dot at location (B) only. Re-install the load center cover (indoor) or trim (outdoor) using screws retained in step 2. Torque screws to 20 lb-in. (2.3 N•m). 11. Re-energize the load center. E-ONE has decided that a defect which relates to motor vehicle safety exists in certain models of the following emergency vehicles manufactured February 2020 – January 2022, equipped with Square D QO Plug-On Neutral Load Centers: E-ONE – 2020 Commercial, 2019-2021 Cyclone 2, 2021 Cyclone N, 2020 Quest 2, 2020-2021 Typhoon, 2020-2022 Typhoon N WHY IS A RECALL BEING CONDUCTED? As part of Schneider Electric’s ongoing quality monitoring process, a potential issue was detected that may affect specific QO Plug-On Neutral Load Centers manufactured between February 1, 2020 and January 12, 2022 at Schneider Electric Plant 15. The issue detected is a loose neutral screw connection within the QO Plug-On Neutral Load Center. If that connection is loose, the affected load center can overheat, increasing the risk of a fire. Refer to Schneider Electric’s recall number 22-159 at cpsc.gov. The load center can overheat, increasing the risk of a fire. There is a potential risk for property damage and personal injury. There may be no warning that precedes this. The cause is a loose neutral screw connection within the QO Plug-On Neutral Load Center. The affected item is the Square D QO Plug-On Neutral Load Centers. WHAT ARE WE DOING ABOUT THE PROBLEM? Schneider Electric will notify affected customers and ask them to inspect product to determine if it’s affected by the recall. All purchasers and installers should immediately contact Schneider Electric to arrange to have the affected recalled load centers inspected by trained electricians to determine if replacement or repair is required. Affected OEMs will notify vehicle purchasers. The inspection and any resulting replacement or repair are free of charge by Schneider Electric. This will be covered under the normal warranty process.
