Summary
Alamo Group Inc (Alamo) is recalling certain 2019-2022 All JetVac AJV1215, AJV1015, AJV0810, AJV1010, AJV0812, AJV0720, AJV0610, AJV1515, and AJV1215 Recycler vehicles. The clevis attaching the hose reel tilt rod and cylinder to the hose reel is an insufficient length, which can cause damage to the clevis and result in the hose reel falling to the ground.
Remedy
Dealers will install a threaded clevis of proper length, free of charge. Owner notification letters were mailed on October 13, 2022. Owners may contact Alamo’s customer service at 1-800-382-8302.

REASON FOR THIS SAFETY RECALL:
On September 6, 2022, Alamo Group Inc., filed a Defect Information Report with the National Highway Traffic Safety Administration (NHTSA) informing the agency that in AllJetVac vehicles manufactured by Gradall, the clevis which attached the hose reel tilt cylinder rod to the hose reel is not the proper length to allow for full engagement. If the hose reel cylinder rod is not properly engaged with the clevis, the hose reel could tilt forward while the vehicle is stationary, increasing the risk of injury to individuals.
REMEDY:
Remedy parts are available now. Dealers will replace the shortened clevis with a lengthened clevis which will allow for proper engagement with the hose reel tilt cylinder rod. This service must be performed on all affected vehicles at no charge to the vehicle owner. Attached please find the service procedure for completing this repair. The repair takes about 1 hour to complete.
SERVICE PROCEDURE FOR RECALL 22V-669
Gradall Industries has determined that a potential defect may exist with the front mounted hose reel tilt cylinder (P/N 2130120013) on its AllJetVac and AllJetVac Recycler Models manufactured from September 10, 2019 through June 30, 2022. The models included in this recall are AJV1515, AJV1215, AJV1215 Recycler, AJV1015, AJV1010, AJV0812, AJV0810, AJV0720, and AJV0610.
The hose reel tilt cylinder is equipped with a clevis that is threaded onto the rod end of the cylinder for adjustment purposes. On certain vehicles the hose reel tilt cylinder clevis may not have adequate thread engagement onto the cylinder rod. As a result, damage to the clevis and/or cylinder rod threads may occur during travel and normal jetting operations.
While lowering the hose reel to perform maintenance in the engine compartment, the clevis may become disengaged from the rod end of the cylinder causing the hose reel to pivot forward uncontrollably and free fall to the ground. If the clevis disengages, a potential crushing hazard may exist when disengaging the pins and clamps or raising/lowering the hose reel.
Affected Units: All front hose reel mounted Vacall AllJetVac and AllJetVac Recyclers manufactured from September 10, 2019 through June 30, 2022. Included with this bulletin is a list of affected vehicles identified by Gradall/Vacall Serial Number, PWO Number, and chassis VIN. If you are not the current owner of the identified unit(s) please forward this information to the owner or lessee.
All front mounted hose reel vehicles manufactured after June 30, 2022 have been equipped with the required updated clevises.
Corrective Action: The clevis on the hose reel tilt cylinder (P/N 2130120013) on the affected vehicles must be removed and replaced with an updated clevis (P/N VA0000S6257).
The original clevis (1) measures approximately 4 ยพโ in overall length. The updated clevis (2) measures approximately 5 ยฝโ in overall length and has a spacer welded between the clevis and threaded collar.
Chronology :
In May 2022, Gradall was notified by a customer that a hose reel tilt cylinder rod on an AllJetVac vehicle failed while the vehicle was stationary and the hose reel was tilted forward to enable mechanics to perform maintenance on the vehicle.
In June 2022, Gradall was notified by another customer that this same issue had occurred on another AllJetVac vehicle while the vehicle was stationary for maintenance work and the hose reel was tilted forward.
In July 2022, Gradall conducted a safety investigation to assess the failure mode, the failure rate, and potential causes. Gradall reviewed information reported by the two customers and examined the cylinders from the customersโ vehicles. Gradall observed the worn threading on the clevis and measured and compared the clevis used before the change in supplier in 2019 to the current clevis. Gradall also inspected the clevises in vehicles in production.
As of August 29, 2022, Alamo was aware of no warranty claims, no deaths, and no injuries related to this issue. The customer notifications in May and June 2022 are the only such notifications Alamo has received on this issue. On August 29, 2022, Alamo decided to declare a voluntary safety recall to remedy this issue.

Park the vehicle on a flat level surface. Ensure the hose reel clamps (1) are engaged and pins (2) are installed on both sides of the hose reel. If the hose reel clamps are not engaged and the pins are not installed do not attempt to replace the clevis. To latch the hose reel clamps and/or install the pins, the hose reel may need to be raised up using either the engine and hydraulic system or the emergency power pack on the truck. 2. After verifying the hose reel clamps are engaged and pins are installed, chock the wheels, and lock out and tag out the vehicle. 3. Support the front of the hose reel with either a crane, jack stand, or blocks of wood to prevent the hose reel from pivoting downward when removing and replacing the cylinder clevis. 4. After ensuring the hose reel is properly supported, loosen the hydraulic hose on the barrel end of the cylinder to bleed off any pressure. Catch any hydraulic oil that runs out of the connection using an appropriate container. Itโs not necessary to completely remove the hose from the cylinder. 5. Hinge the cylinder downward. Remove the clevis from the cylinder rod by first removing the set screw on the threaded part of the clevis. 7. Inspect the threads on the rod end of the cylinder for damage. If the threads on the cylinder rod are damaged a new cylinder must be installed. If the threads are not damaged, install an updated clevis (P/N VA0000S6257) by threading the clevis completely onto the cylinder rod and securing the set screw. 8. Reinstall the cylinder. It will be necessary to use a prybar to collapse the cylinder about ยพโ before installing the pin, washer, and cotter pin to secure the cylinder. 9. Tighten the hydraulic line going to the cylinder. 10. Remove the blocking and tilt the hose reel up using either the engine and hydraulic system or the emergency electric power pack. Disengage the hose reel clamps and pins. Keep clear of the front of the hose reel when disengaging the clamps and pins as any air trapped in the hydraulic circuit may cause the hose reel to drop slightly. It may be necessary to operate the hose reel tilt up and down throughout the cylinderโs entire range to eliminate the air in the hydraulic circuit. 11. Unless access to the engine compartment is required, engage the hose reel clamps, and install the pins on each side prior to travel or jetting operations. Ensure the pins are installed and the clamps are engaged and tight while driving or using the vehicle for jetting operations.

Description of Remedy : Description of Remedy Program : Gradall will provide the customer with a threaded clevis of proper length to allow for proper engagement between the hose reel tilt cylinder rod and the new clevis. Affected vehicle owners will be notified by first class mail to take their vehicle to a Gradall affiliated dealer to have the remedy performed free of charge. If this condition was identified and remedied on an affected machine prior to this campaign, Gradall will reimburse owners who have incurred the cost of such remedy. How Remedy Component Differs from Recalled Component : The remedy component has the additional length required for proper thread engagement with the hose reel tilt cylinder rod. Once detached, the hose reel tilt cylinder rod could fail to support the hose reel, causing the hose reel to over-tilt forward and make contact with the ground, increasing the risk of injury to a person nearby. Component Part Number : 2130120013 Supplier Identification : Component Manufacturer Name : Bailey International, LLC Address : 2527 Westcott Blvd. Knoxville Tennessee 37931 Country : United States Chronology : In May 2022, Gradall was notified by a customer that a hose reel tilt cylinder rod on an AllJetVac vehicle failed while the vehicle was stationary and the hose reel was tilted forward to enable mechanics to perform maintenance on the vehicle. In June 2022, Gradall was notified by another customer that this same issue had occurred on another AllJetVac vehicle while the vehicle was stationary for maintenance work and the hose reel was tilted forward. In July 2022, Gradall conducted a safety investigation to assess the failure mode, the failure rate, and potential causes. Gradall reviewed information reported by the two customers and examined the cylinders from the customersโ vehicles. Gradall observed the worn threading on the clevis and measured and compared the clevis used before the change in supplier in 2019 to the current clevis. Gradall also inspected the clevises in vehicles in production. As of August 29, 2022, Alamo was aware of no warranty claims, no deaths, and no injuries related to this issue. The customer notifications in May and June 2022 are the only such notifications Alamo has received on this issue. On August 29, 2022, Alamo decided to declare a voluntary safety recall to remedy this issue. Part 573 Safety Recall Report 22V-669 Page 3 The information contained in this report was submitted pursuant to 49 CFR ยง573 Description of Remedy : Description of Remedy Program : Gradall will provide the customer with a threaded clevis of proper length to allow for proper engagement between the hose reel tilt cylinder rod and the new clevis. This remedy will be performed free of charge. Affected vehicle owners will be notified by first class mail to take their vehicle to a Gradall affiliated dealer to have the remedy performed free of charge. If this condition was identified and remedied on an affected machine prior to this campaign, Gradall will reimburse owners who have incurred the cost of such remedy. How Remedy Component Differs from Recalled Component : The remedy component has the additional length required for proper thread engagement with the hose reel tilt cylinder rod. Identify How/When Recall Condition was Corrected in Production : As of August 29, 2022, Gradall has worked with the supplier to provide longer clevises that allow for proper thread engagement with the hose reel tilt cylinder rod. These longer clevises will be used on all machines produced. The two units identified by customers who were performing engine maintenance have also been remedied by Gradall.
