Chronology :
On August 16, 2022, Carrier received notification of a thermal incident involving a Supra S6 TRU in Aurora, CO.
On September 14, 2022, a field inspection was conducted, with an independent third-party fire investigator, into potential causes of the thermal event.
On September 20, 2022, a production line inspection showed that the affected TRU’s wire harness was missing the heat shrinks, such that the exposed wires in the harness may come into contact with surrounding metal or other exposed wires.
As a result, all shipments of affected TRUs were put on hold on September 22, 2022, and Carrier immediately began a careful analysis of the affected TRU’s wire harness to assess the potential product safety risk.
To date there has been only the one thermal event related to this issue, and no other field reports of arcing.
After much internal analysis and discussion with safety engineers, on October 20, 2022, Carrier determined that the issue presented a potential safety-related defect reportable to NHTSA under Part 573. That same business day, on October 20, 2022, Carrier
filed this 573 Defect and Non-Compliance Report.
