Summary
Schwarze Industries, Inc. (Schwarze) is recalling certain 2012-2020 Avalanche M6TE, M6SE, and M6SE CNG street sweepers. The outside switch box that controls the position of the conveyor may activate unintentionally while the vehicle is running.
Remedy
Dealers will install a kit containing an outside switch guard and two crushing hazard warning decals, free of charge. Owner notification letters were mailed on November 23, 2021. Owners may contact Schwarze’s customer service at 1-800-879-7934.

Avalanche M6TE, M6SE & M6SE CNG Models
This Switch Guard/DANGER/WARNING decal kit has been developed to prevent unintended activation of the outside controls and to warn all persons to remain clear of the area between the hopper and conveyor of energized sweepers.
Note: Always shut off all engines, deactivate equipment, and remove keys anytime inspecting or servicing the sweeper. Never step on the switch control box or switch guard under any circumstances.
Updates to Outside Controls: Kit (204Z16)
- Addition of Switch Guard (651326) to the face of Switch Control Box as described on page 2 of this bulletin.
- Addition of WARNING decal with part number (68916) to the top of Switch Control Box as shown on page 2 of this bulletin.
Updates to Each Side of Conveyor Opening on Hopper:
- Addition of two (2) DANGER decals with part number (68915), one (1) decal on each side of conveyor opening on hopper as shown on pages 3 and 4 of this bulletin.
Switch Guard Installation and WARNING Decal Placement
- Remove (4) four plastic fasteners from front of Switch Control Box’s (4) corner holes.
- Using a 3/16” drill bit, open up pass through holes in (4) corner holes, from the back.
- Insert (4) 10-24 x 3-1/2” socket cap screws into the (4) corner holes, from the back.
- Place Switch Guard (651326) in place over (4) bolts with flat washers, add hex lock nuts and tighten using a 3/8” socket and 5/32” Allen and 5/32” Allen wrench.
- Clean top surface of Switch Control Clean top surface of Switch Control Box.

MYs 2012 – 2020 Avalanche M6TE, M6SE, and M6SE CNG
Outside Switch Box
REASON FOR THIS SAFETY RECALL:
On October 1 8, Schwarze Industries, Inc., filed a Defect Information Report with the National Highway Traffic Safety Administration (NHTSA) informing the agency that in certain Avalanche M6 sweepers, the outside switch box, from which the operator can control the position of the conveyor, can be activated unintentionally when the vehicle is running. If the vehicle operator has not completed lock out/tag out procedures and not installed the conveyor safety pins to keep the retracted conveyor in place, but enters the area between the hopper and the conveyor, the operator can unintentionally activate the switch that advances the conveyor, thereby pinning the operator between the hopper and the conveyor. This presents a crushing hazard, increasing the risk of serious injury or death.

REMEDY:
Remedy parts are available. Dealers will install an Outside Switch Guard and two crushing hazard Warning Decals. This service must be performed on all affected vehicles at no charge to the vehicle owner. Attached please find the service procedure for completing this repair. The repair takes about 30 minutes to complete.
REIMBURSENT:
Schwarze will provide the remedy kit to dealers at no charge and will reimburse dealers directly for performing this remedy.
OWNER LETTER MAILING DATE:
Schwarze will notify owners of this safety recall by December 17, 2021.
CUSTOMER CONTACTS:
Dealers should repair any Avalanche M6 sweeper covered by this safety recall that arrives at their dealerships, whether or not the owner has received a safety recall letter. The Schwarze Warranty Department (at 800-879-7934 or warranty@schwarze.com) is available to support you in executing a customer-friendly process for remedying these vehicles.
NEW VEHICLES IN DEALER NVENTORY:
Schwarze does not believe there are any covered vehicles in dealers’ inventory. However, it is a violation of Federal law for a dealer to deliver a new motor vehicle or any new or used item of motor vehicle equipment (including a tire) covered by this notification under a sale or lease until the defect is remedied. Violation of this requirement by a dealer could result in a civil penalty against the dealer of up to $22,723 per vehicle. Accordingly, you must remedy covered vehicles in your new vehicle inventory before delivery.
In addition, we request that you remedy before delivery into commerce under a sale or lease any covered pre-owned vehicles in your inventory

Chronology :
On September 7, 2017, Schwarze learned of a fatality involving an M6 sweeper. A Schwarze employee’s initial inspection that day and Schwarze’s further investigation did not identify any malfunction in the vehicle.
On August 10, 2020, Schwarze learned of a fatality involving an M6 sweeper.
On August 20, 2020, Schwarze attended an inspection of the vehicle.
On September 22, 2020, Schwarze produced an initial design of a switch box guard.
On November 3, 2020, Schwarz produced the final design of the switch box guard.
On November 10, 2020, Schwarze participated in a follow-up vehicle inspection.
On December 15, 2020, Schwarze began installing the new switch box guard and crushing hazard warning decals on all M6 sweepers and began retrofitting existing units (manufactured in 2012 and thereafter) with the switch guard box and crushing hazard warning decals.
On September 10 and October 8, 2021, Schwarze met with NHTSA on the 2017 and 2020 sweeper incidents.
On October 8, 2021, Schwarze decided to conduct a voluntary safety recall to install the switch guard box and crushing hazard warning decals on the remainder of the affected M6 sweeper population. As of October 8, 2021, Schwarze is aware of no warranty claims, no field or service reports, and no injuries related to this issue.

General Plan for Reimbursement of Pre-Notification Remedy Costs In accordance with the requirements of 49 CFR §§ 573.6, 573.13 and 577.11, Alamo Group Inc., hereby submits to the National Highway Traffic Safety Administration the required information for Alamo Group’s general plan to reimburse owners and purchasers for costs incurred for remedies in advance of notification of a safety-related defect or noncompliance with Federal motor vehicle safety standards under 49 CFR 573.6. Alamo Group will implement this plan, in accordance with 49 CFR 573.13(k). 1. Terms of Reimbursement: Alamo Group will reimburse owners for costs of pre-notification remedies, subject to the following terms: a. Excluded from reimbursement are: i. Any costs incurred during the period in which an original or extended warranty from Alamo Group would have provided for a free repair of the problem identified in the recall, in accordance with 49 CFR 573.13(d)(1). ii. Any instances where, in accordance with 49 CFR 573.13(d)(2), the pre-notification remedy: I. was not of the same type as the recall remedy; II. did not address the defect or noncompliance that led to the recall or a manifestation of the defect or noncompliance, or III. was not reasonably necessary to correct the defect or noncompliance that led to the recall or a manifestation of the defect or noncompliance; and iii. Any instance where, in accordance with 49 CFR 573.13(d)(4), the claimant did not submit adequate documentation to Alamo Group at the address listed under b, below. b. Claimants should mail reimbursement claims to the following address: Alamo Group Inc., Attn: Andrew Sefzik, 1627 E. Walnut, Seguin, Texas 78155 c. Alamo Group will act upon any adequate and properly submitted request for reimbursement within 60 days of its receipt, including notifying of any denial of claim or any incomplete claim and opportunity to resubmit, in accordance with 49 CFR 573.13(g). d. Reimbursement shall be in the form of a check or cash from Alamo Group or a designated dealer or facility. e. In accordance with 49 CFR 573.13(n), this reimbursement plan does not apply if a motor vehicle was bought by the first purchaser more than 10 calendar years before Alamo Group’s notification to NHTSA of a defect or noncompliance in accordance with 49 CFR 573.6 or NHTSA’s order determining a safety related defect or noncompliance exists in Alamo Group’s products under 49 USC 31108(b). 2. Documentation Required The following documentation must be submitted to Alamo Group in support of any request for reimbursement of pre-notification remedy costs, in accordance with 49 CFR 573.13(d)(4): a. Name and address of the claimant; b. Identification of the vehicle by make, model, model year, and vehicle identification number; c. Identification of the recall by NHTSA recall number; d. Identification of the owner or purchaser of the recalled motor vehicle at the time the pre-notification remedy was obtained; e. A receipt for the pre-notification or remedy, either an original or a copy, and indicating that the repair addressed the defect leading to the recall, in accordance with 49 CFR 573.13(d)(4)(v); and f. If the remedy was obtained at a time when the vehicle could have been repaired or replaced at no charge under Alamo Group’s original or extended warranty program, documentation indicating that the Alamo Group dealer or authorized facility either refused to remedy the problem addressed by the recall under warranty or that the warranty repair did not correct the problem addressed by the recall 3. Amount of Costs to be Reimbursed: The amount of reimbursement shall be not less than the lesser of (1) the amount paid by the owner for the remedy, or (2) the cost of remedy parts, associated labor, and, without limit, miscellaneous fees such as disposal of waste and taxes. 4. Period for Reimbursement: In accordance with 49 CFR 573.13(c), Alamo Group will adhere to the following beginning and end dates for the reimbursement period under this plan: a. Beginning Date: i. For a noncompliance, the reimbursement period will start no later than the date of the first test or observation indicating a noncompliance may exist. ii. For a defect determination resulting from a NHTSA Engineering Analysis, the reimbursement period will start no later than the date the Engineering Analysis was opened, or one year before the date of Alamo Group’s notification of a defect to NHTSA under 49 CFR 573.6, whichever is earlier. iii. For all other defect determinations, the reimbursement period will start no later than one year before the date of Alamo Group’s notification of a defect to NHTSA under 49 CFR 573.6. b. Ending Date: The reimbursement period will end no earlier than 10 calendar days after the date on which Alamo Group mailed the last owner notification letter pursuant to 49 CFR Part 577. 5. Additional Terms of the Plan: a. Alamo Group will make this reimbursement plan available to the public upon request. b. Any disputes of the denial of a claim for reimbursement shall be resolved between the claimant and Alamo Group, and NHTSA will not mediate or resolve any such disputes. c. This plan shall not apply to recalls based solely on noncompliant or defective labels. d. Nothing in this plan requires Alamo Group to provide reimbursement in connection with a fraudulent claim for reimbursement.

The outside switch box, from which the operator can control the position of the conveyor, can be activated unintentionally when the vehicle is running. If the vehicle operator has not completed lock out/tag out procedures and not installed the conveyor safety pins to keep the retracted conveyor in place, but enters the area between the hopper and the conveyor, the operator can unintentionally activate the switch that advances the conveyor, thereby pinning the operator between the hopper and the conveyor. This presents a crushing hazard, increasing the risk of serious injury or death. A remedy is available now. Schwarze has developed an Outside Switch Guard and two crushing hazard W aming Decals for these M6 Avalanche street sweepers. You may install this remedy kit yourself or contact your local Schwarze dealer, who will perform the installation free of charge. This repair takes about 30 minutes to complete. November 2021 To: All Schwarze-affiliated Dealers Subject: Notification of Safety Recall 21 V-816, New Vehicle Hold MYs 2012 – 2020 Avalanche M61E, M6SE, and M6SE CNG Outside Switch Box REASON FOR THIS SAFETY RECALL: On October 18, Schwarze Industries, Inc., filed a Defect Information Report with the National Highway Traffic Safety Administration (NHTSA) informing the agency that in certain Avalanche M6 sweepers, the outside switch box, from which the operator can control the position of the conveyor, can be activated unintentionally when the vehicle is running. If the vehicle operator has not completed lock out/tag out procedures and not installed the conveyor safety pins to keep the retracted conveyor in place, but enters the area between the hopper and the conveyor, the operator can unintentionally activate the switch that advances the conveyor, thereby pinning the operator between the hopper and the conveyor. Dealers will install an Outside Switch Guard and two crushing hazard Warning Decals. This service must be performed on all affected vehicles at no charge to the vehicle owner. Attached please find the service procedure for completing this repair. The repair takes about 30 minutes to complete. REIMBURSEMENT: Schwarze will provide the remedy kit to dealers at no charge and will reimburse dealers directly for performing this remedy. OWNER LETTER MAILING DATE: Schwarze will notify owners of this safety recall by December 17, 2021. CUSTOMER CONTACTS: Dealers should repair any Avalanche M6 sweeper covered by this safety recall that arrives at their dealerships, whether or not the owner has received a safety recall letter. NEW VEIDCLES IN DEALER INVENTORY: Schwarze does not believe there are any covered vehicles in dealers’ inventory. However, it is a violation of Federal law for a dealer to deliver a new motor vehicle or any new or used item of motor vehicle equipment (including a tire) covered by this notification under a sale or lease until the defect is remedied. Violation of this requirement by a dealer could result in a civil penalty against the dealer of up to $22,723 per vehicle. Accordingly, you must remedy covered vehicles in your new vehicle inventory before delivery.
