Fuel Tubes May Crack and Leak Fuel
A high pressure fuel leak in the presence of an ignition source can increase the risk of a fire.

Summary
HME, Inc. (HME) is recalling certain 2019-2021 AF1 and 1871 vehicles. The fuel tubes between the fuel rail and the injectors for cylinders four, five, and six may fatigue and crack, which can result in a high pressure fuel leak.
Remedy
Dealers will install new fuel tubes with vibration isolators, free of charge. The manufacturer has not yet provided a schedule for recall notification. Owners may contact HME customer service at 1-616-534-1463.

19Apr2021: Cummins received a Technical Service Request for a leaking fuel tube on an X12 engine.
19Apr-23Jul2021: Cummins received additional reports of leaking fuel tubes on X12 engines and escalated the issue for further investigation.
23Jul2021: Cummins initiated a Product Problem Solving (PPS) project to investigate the cause of the fuel tube leaks.
Aug-Oct2021: Cummins continued to investigate this issue, including obtaining and analyzing parts from the field, conducting testing to understand the cause of the failures and evaluating potential corrective action.
19Oct2021: Cummins investigated a September 2021 fire on a vehicle equipped with an X12 engine.
25Oct2021: Cummins identified a leak of the #5 cylinder fuel tube as the cause of the fire.
28Oct2021: Cummins completed a Product Safety Hazard Analysis.
1Nov-23Nov2021: Cummins escalated the issue through its Product Safety Defect Board process.
23Nov2021: Based upon results of the investigation, the Cummins Product Safety Defect Board decided to conduct a safety campaign to address this condition.
7Dec2021: Cummins notified HME of safety campaign.
Dec2021: HME worked with Cummins to verify current inventory of uninstalled engines and which needed to be remedied.
DEC2021: Affected uninstalled engines are repaired by Cummins.
Dec2021: HME provides owner names and addresses for notification letters, which Cummins will send.
Jan-Feb2022: HME transitions responsibility for NHSTA related items and training starts.

This notice applies to the engines(s)/vehicles(s) listed on the enclosed table. You previously received a letter informing you of this recall and advising you that Cummins was developing a remedy. A remedy is now available for your engine/vehicle. WHAT IS THE PROBLEM? The fuel tubes between the fuel rail and the injectors for cylinders 4, 5 and 6 may be susceptible to cracking, resulting in a high pressure fuel leak. A high pressure fuel leak in the presence of an ignition source may increase the risk of a fire. WHAT ARE WE DOING ABOUT THE PROBLEM? Cummins has released field Safety Campaign #C2532, X12 CM2350 X119B and X12 CM2450 X137B Fuel Injector Supply Line Safety Campaign to correct this condition by replacing the fuel tubes with new fuel tubes and installing vibration isolators for cylinders 4,5 and 6. This campaign is currently available at Cummins Distributors and authorized Warranty dealers. The repair will be completed free of charge and most applications will require approximately 2 hours to complete. WHAT SHOULD YOU DO? Contact the nearest Cummins Distributor or authorized Warranty Dealer to arrange to have this campaign performed on your engine. The distributor or dealer will work with you to schedule the best date to complete the repair. In the meantime, Cummins advises you to periodically check the injector fuel tubes for signs of leaking and to seek repairs if leaks are identified.
