Columbus Trading-Partners USA, Inc. (CTP) is recalling certain CYBEX Sirona M Convertible Child Car Seat, model numbers 518000385, 518002153, 518000387, 518002145, 518002149, 518002151, and 519000211, manufactured between November 3, 2017 and August 31, 2018. A child may pick pieces of foam from the child seat
headrest pad.
Remedy
CTP will mail a kit with instructions on how to seal the headrest foam, free of charge. Owner notification letters were mailed May 23, 2022. Owners may contact CTP customer service at 1-877-242-5676.

Cybexยฎ will offer a free product improvement kit to owners of certain US versions of the Sirona M child restraints manufactured before September 1, 2018. Cybex discovered that some children can access the foam headrest through a small opening in the headrest cover set that covers the foam. While seated in the car seat, a child may pick off small pieces of foam and place that foam in their mouth, nose, or ears, posing a potential choking hazard. The foam is non-toxic and is a small, spherical shape with no sharp edges. While there have been no reports of serious injury, in an abundance of caution and commitment to safety and continuous product improvement, Cybex elected to conduct a voluntary recall.
The product improvement kit, consisting of written instructions and manufacturer-approved tape to apply to the foam, is only needed for US versions of Sirona M models manufactured before September 1, 2018 with model numbers 518000385, 518000387, 518002145, 518002149, 518002151, 518002153 and 519000211. The model number and date of manufacture are located on the bottom of the seat base.

Please be advised that CYBEX and Columbus Trading-Partners U.S., Inc. (โCTPโ), the United States distributor for CYBEX (collectively โCYBEXโ) have decided, as of April 8, 2022, to conduct a voluntary recall on select U.S. versions of the Sirona M models manufactured prior to September 1, 2018 (see table for details).
The recall of these select U.S. versions of the Sirona M convertible car seats is due to some children accessing and picking off pieces of foam from the headrest through a small opening in the headrest cover set. The foam pieces are very small, generally spherical with no sharp edges, and non-toxic. However, these small foam pieces could be a choking hazard. There have been no reports to CYBEX of injuries to children in connection with the foam the U.S. Sirona M. The car seat
continues to meet all requirements for crashworthiness. The affected U.S. versions of the Sirona M car seat
can continue to be used to transport children safely.
The company will send a consumer notice to registered owners of the affected seats on or about May 9, 2022 to inform them of the action and is working with retailers to ensure that impacted U.S. versions of the Sirona M car seats are no longer available for sale. CYBEX will provide a free product improvement kit that includes manufacturer-approved adhesive tape and instructions for proper application.
The company requests that owners of affected seats contact CYBEX for a free product improvement kit by calling 1-877-242-5676 between 8 am and 5 pm ET, Monday through Friday. Customers should not return seats to retailers where purchased. Importantly, the U.S. versions of the Sirona M car seats affected by this recall meet all safety requirements, so they may be used while consumers wait for the product improvement kit.
According to CYBEXโs records, you should no longer have the affected product in your inventory. None of the U.S. versions of Sirona M car seats shipped or manufactured since September 1, 2018 are subject to this recall.

Sirona M Remedy Kit
1. Remove headrest cover.
2. Line up 135mm guide with top of center foam panel, and mark 135mm from top of center foam panel.
3. Line up one piece of tape with the mark and with space in between foam panels, and press onto side foam panel.
4. Wrap tape under side foam panel and press to secure.
5. Slightly overlap second piece of tape onto the first, and press onto side foam panel.
6. Wrap tape under side foam panel and press to secure.
7. Repeat for other side.
8. Center foam panel: align tape with the top of the harness slot and the second rib from outside and press into center foam panel.
9. Center foam panel: align final tape with top of harness slot and second rib from outside and press into center foam panel.
10. Center foam panel: wrap both tape pieces under center foam panel and press to secure.
11. Replace headrest cover.

Chronology :
CTP closely tracked feedback from consumer owners of the US version of the CYBEX Sirona M following the release of the car seat in the U.S. market. Early in 2018, CTP received a few reports that children could access the foam under the seat pad in the headrest area and could and/or did place the small foam pieces in their mouths.
To address consumer feedback and to continuously improve our products for our consumers, CTP implemented an engineering change on August 31, 2018, which consisted of adding tape to the lower edges of the EPS in the child restraintโs headrest. This eliminated the ability of a child from being able to pick or scrape the EPS.
NHTSA submitted a request for information to CTP on July 16, 2020 related to two reports of foam picking on the car seat. CTP responded to NHTSA providing information regarding the engineering change. CTP remained in communication with NHTSA regarding the foam picking, providing NHTSA additional requested information on September 20, 2021 and December 8, 2021.
While CTP received reports between early 2018 and November 2021 that 53 children could or possibly did pick foam and possibly place foam pieces in their mouths, CTP did not receive any reports of any injuries or fatalities. The pieces of foam are small, soft, and non-toxic. However, if a child picks a piece of foam and places it into their mouth, nose, or ear the foam could pose a choking hazard or otherwise cause a safety concern for caregivers.
Following additional discussions with NHTSA and in furtherance of continuous product improvement and a robust commitment to safety, CTP determined it should prepare and provide a product improvement kit to consumers through a Voluntary Safety Recall Campaign which will enable consumers who own certain US Sirona M models manufactured on or before August 31, 2018 to apply tape to the EPS foam to prevent foam picking.

Chocking on / Ingestion of Car Seat Foam

The National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation (ODI) Trends Analysis Division identified 15 Early Warning Reporting (EWR) Death & Injury (D&I) reports alleging injuries involving production year 2018 Cybex Sirona M car (child) seats. According to the reports, child occupants were able to access a foam material, used in the seat headrest, via an opening in its covering material. The children subsequently placed the foam in their mouths thereby resulting in coughing, gagging, vomiting, or passed by means of bowel movements. No fatalities have been reported.
In addition to the D&I reports, ODI has received four Vehicle Owner Questionnaire (VOQ) reports from two complainants involving the 2018 Cybex Sirona M car seat. Like the D&I reports, the VOQs also express concern for the same alleged defect, that the foam in the car seat
headrest was accessible to child occupants through its covering material leading to potential ingestion. Three of the four reports were submitted by the same complainant who owned two subject car seats
. No injuries are alleged in these reports.
ODI is opening this investigation to evaluate the scope, frequency, and consequence of the alleged defect in the subject 2018 Cybex Sirona M car seat. The VOQs cited above can be reviewed at NHTSA.gov under the reference (ODI) numbers 11113133 and 11150030. VOQs 11150031 & 11150032 are duplicative of ODI 11150030.

518002145 Platform Name / No. : NR Brand Name : CYBEX Descriptive Information : The US version of the CYBEX Sirona M is a convertible child restraint system that is used both rear-facing and forward-facing that is distributed by Columbus Trading-Partners (CTP). The structure of the US Sirona M is primarily steel and rigid plastic. However, foam is used in child-contactable areas, including the headrest, for safety and comfort purposes. The foam used is expanded polystyrene (EPS) with a composition small beads (3-4 mm) bonded together to form a rigid structure. A small number of consumers have notified CTP and NHTSA that their child has accessed and picked small pieces of the foam. Based on these reports, CTP initially implemented an engineering change on August 31, 2018 to add tape to the lower edges of the EPS to prevent access to the foam. Description of the Defect : A small number of consumers notified CTP and NHTSA that is possible for a child to access and pick small pieces of foam under the headrest pad on the US version of the Cybex Sirona M convertible seat. It has been reported that a child can put the small piece of foam into their mouth. These reports have not included any claims of injury or choking. Moreover, the foam picking does not impact the crashworthiness of the seat and the seat is in all other ways compliant with FMVSS 213. However, CTP has determined that a child can access the foam through a small opening in the headrest fabric that covers the foam headrest. It has been reported that a child can pick small pieces of foam which could pose a choking hazard. Pictures of the car seats showing the headrest fabric and headrest foam are included in the CTP response letters to NHTSA dated 7.22.20, 9.20.21 and 12.8.21 in connection with NHTSA inquiry NEF 0102SW/ PE-21-014. The foam pieces that were picked are small (roughly 3-4 mm), generally spherical pieces, with no sharp edges. The foam is non-toxic and does not pose a chemical or poison concern if ingested. CTP closely tracked feedback from consumer owners of the US version of the CYBEX Sirona M following the release of the car seat in the U.S. market. Early in 2018, CTP received a few reports that children could access the foam under the seat pad in the headrest area and could and/or did place the small foam pieces in their mouths. To address consumer feedback and to continuously improve our products for our consumers, CTP implemented an engineering change on August 31, 2018, which consisted of adding tape to the lower edges of the EPS in the child restraintโs headrest. This eliminated the ability of a child from being able to pick or scrape the EPS. NHTSA submitted a request for information to CTP on July 16, 2020 related to two reports of foam picking on the car seat. CTP responded to NHTSA providing information regarding the engineering change. CTP remained in communication with NHTSA regarding the foam picking, providing NHTSA additional requested information on September 20, 2021 and December 8, 2021.
While CTP received reports between early 2018 and November 2021 that 53 children could or possibly did pick foam and possibly place foam pieces in their mouths, CTP did not receive any reports of any injuries or fatalities. The pieces of foam are small, soft, and non-toxic. However, if a child picks a piece of foam and places it into their mouth, nose, or ear the foam could pose a choking hazard or otherwise cause a safety concern for caregivers. Following additional discussions with NHTSA and in furtherance of continuous product improvement and a The information contained in this report was submitted pursuant to 49 CFR ยง573 Part 573 Safety Recall Report 22C-001 Page 10 robust commitment to safety, CTP determined it should prepare and provide a product improvement kit to consumers through a Voluntary Safety Recall Campaign which will enable consumers who own certain US Sirona M models manufactured on or before August 31, 2018 to apply tape to the EPS foam to prevent foam picking. Description of Remedy :
Description of Remedy Program : CTP plans to send a product improvement kit to owners of the impacted
seats consisting of manufacturer-approved tape and instructions to apply the tape to the EPS headrest foam. The tape will eliminate access to the foam by the occupant. CTP plans to complete the following activities:
1. CTP will mail a notification letter to all registered consumers of the impacted US Sirona M car seats informing them about the potential foam picking issue and enclosing manufacturer-approved tape and instructions for properly applying the tape to prevent access to the headrest foam.
2. CTP will issue a press release on its website and will issue communications on its social media accounts with information about the safety recall campaign and available product improvement kit. Owners will be instructed how to find the model number and date of manufacture on their seat(s). CTP will provide owners of US Sirona M seats manufactured on or before August 31, 2018 who contact CTP with a product improvement kit including manufacturer-approved tape to cover the EPS foam and instructions for proper application to prevent access to the foam. CTP will also advise consumers that all US Sirona M seats manufactured after August 31, 2018 come with the additional tape already applied preventing access to the EPS headrest foam and are not the subject of the recall campaign.
3. CTP will put a video on its website demonstrating proper application of the manufacturer-approved tape to the EPS headrest foam. All notification letters sent to consumers will include a QR code to link them to this video as well as a website address where the video is located. How Remedy Component Differs from Recalled Component : Identify How/When Recall Condition was Corrected in Production : The manufacturer-approved tape will be accompanied by clear written instructions, a QR code linking the consumer to a video demonstrating the proper application of the tape, and a website address where the video instructions can also be found. These instructions will assist the consumer in clearly distinguishing the tape from any existing feature or component of the US Sirona M.
As noted above, based on consumer feedback and as a commitment to continuous product improvement, CTP implemented an engineering change on August 31, 2018, which consisted of adding tape to the headrest EPS foam on the US Sirona M to prevent access to the foam. Description of the Defect : A small number of consumers notified CTP and NHTSA that is possible for a child to access and pick small pieces of foam under the headrest pad on the US version of the Cybex Sirona M convertible seat. It has been reported that a child can put the small piece of foam into their mouth. These reports have not included any claims of injury or choking. Moreover, the foam picking does not impact the crashworthiness of the seat and the seat is in all other ways compliant with FMVSS 213. However, CTP has determined that a child can access the foam through a small opening in the headrest fabric that covers the foam headrest. It has been reported that a child can pick small pieces of foam which could pose a choking hazard. Pictures of the car seats showing the headrest fabric and headrest foam are included in the CTP response letters to NHTSA dated 7.22.20, 9.20.21 and 12.8.21 in connection with NHTSA inquiry NEF 0102SW/ PE-21-014. CTP closely tracked feedback from consumer owners of the US version of the CYBEX Sirona M following the release of the car seat in the U.S. market. Early in 2018, CTP received a few reports that children could access the foam under the seat pad in the headrest area and could and/or did place the small foam pieces in their mouths. To address consumer feedback and to continuously improve our products for our consumers, CTP implemented an engineering change on August 31, 2018, which consisted of adding tape to the lower edges of the EPS in the child restraintโs headrest. This eliminated the ability of a child from being able to pick or scrape the EPS. NHTSA submitted a request for information to CTP on July 16, 2020 related to two reports of foam picking on the car seat. CTP responded to NHTSA providing information regarding the engineering change. CTP remained in communication with NHTSA regarding the foam picking, providing NHTSA additional requested information on September 20, 2021 and December 8, 2021.
While CTP received reports between early 2018 and November 2021 that 53 children could or possibly did pick foam and possibly place foam pieces in their mouths, CTP did not receive any reports of any injuries or fatalities. The pieces of foam are small, soft, and non-toxic. However, if a child picks a piece of foam and places it into their mouth, nose, or ear the foam could pose a choking hazard or otherwise cause a safety concern for caregivers. Description of Remedy Program : CTP plans to send a product improvement kit to owners of the impacted
seats consisting of manufacturer-approved tape and instructions to apply the tape to the EPS headrest foam. The tape will eliminate access to the foam by the occupant. CTP plans to complete the following activities:
1. CTP will mail a notification letter to all registered consumers of the impacted US Sirona M car seats informing them about the potential foam picking issue and enclosing manufacturer-approved tape and instructions for properly applying the tape to prevent access to the headrest foam.
2. CTP will issue a press release on its website and will issue communications on its social media accounts with information about the safety recall campaign and available product improvement kit. Owners will be instructed how to find the model number and date of manufacture on their seat(s). CTP will provide owners of US Sirona M seats manufactured on or before August 31, 2018 who contact CTP with a product improvement kit including manufacturer-approved tape to cover the EPS foam and instructions for proper application to prevent access to the foam. CTP will also advise consumers that all US Sirona M seats manufactured after August 31, 2018 come with the additional tape already applied preventing access to the EPS headrest foam and are not the subject of the recall campaign.
3. CTP will put a video on its website demonstrating proper application of the manufacturer-approved tape to the EPS headrest foam. All notification letters sent to consumers will include a QR code to link them to this video as well as a website address where the video is located. How Remedy Component Differs from Recalled Component : Identify How/When Recall Condition was Corrected in Production : The manufacturer-approved tape will be accompanied by clear written instructions, a QR code linking the consumer to a video demonstrating the proper application of the tape, and a website address where the video instructions can also be found. These instructions will assist the consumer in clearly distinguishing the tape from any existing feature or component of the US Sirona M.
As noted above, based on consumer feedback and as a commitment to continuous product improvement, CTP implemented an engineering change on August 31, 2018, which consisted of adding tape to the headrest EPS foam on the US Sirona M to prevent access to the foam. Description of the Defect : A small number of consumers notified CTP and NHTSA that is possible for a child to access and pick small pieces of foam under the headrest pad on the US version of the Cybex Sirona M convertible seat. It has been reported that a child can put the small piece of foam into their mouth. These reports have not included any claims of injury or choking. Moreover, the foam picking does not impact the crashworthiness of the seat and the seat is in all other ways compliant with FMVSS 213. However, CTP has determined that a child can access the foam through a small opening in the headrest fabric that covers the foam headrest. It has been reported that a child can pick small pieces of foam which could pose a choking hazard. Pictures of the car seats showing the headrest fabric and headrest foam are included in the CTP response letters to NHTSA dated 7.22.20, 9.20.21 and 12.8.21 in connection with NHTSA inquiry NEF 0102SW/ PE-21-014. The foam pieces that were picked are small (roughly 3-4 mm), generally spherical pieces, with no sharp edges. The foam is non-toxic and does not pose a chemical or poison concern if ingested. CTP closely tracked feedback from consumer owners of the US version of the CYBEX Sirona M following the release of the car seat in the U.S. market. Early in 2018, CTP received a few reports that children could access the foam under the seat pad in the headrest area and could and/or did place the small foam pieces in their mouths. To address consumer feedback and to continuously improve our products for our consumers, CTP implemented an engineering change on August 31, 2018, which consisted of adding tape to the lower edges of the EPS in the child restraintโs headrest. This eliminated the ability of a child from being able to pick or scrape the EPS. NHTSA submitted a request for information to CTP on July 16, 2020 related to two reports of foam picking on the car seat. CTP responded to NHTSA providing information regarding the engineering change. CTP remained in communication with NHTSA regarding the foam picking, providing NHTSA additional requested information on September 20, 2021 and December 8, 2021.
While CTP received reports between early 2018 and November 2021 that 53 children could or possibly did pick foam and possibly place foam pieces in their mouths, CTP did not receive any reports of any injuries or fatalities. The pieces of foam are small, soft, and non-toxic. However, if a child picks a piece of foam and places it into their mouth, nose, or ear the foam could pose a choking hazard or otherwise cause a safety concern for caregivers. Description of Remedy :
Description of Remedy Program : CTP plans to send a product improvement kit to owners of the impacted
seats consisting of manufacturer-approved tape and instructions to apply the tape to the EPS headrest foam. The tape will eliminate access to the foam by the occupant. CTP plans to complete the following activities:
1. CTP will mail a notification letter to all registered consumers of the impacted US Sirona M car seats informing them about the potential foam picking issue and enclosing manufacturer-approved tape and instructions for properly applying the tape to prevent access to the headrest foam.
2. CTP will issue a press release on its website and will issue communications on its social media accounts with information about the safety recall campaign and available product improvement kit. Owners will be instructed how to find the model number and date of manufacture on their seat(s). CTP will provide owners of US Sirona M seats manufactured on or before August 31, 2018 who contact CTP with a product improvement kit including manufacturer-approved tape to cover the EPS foam and instructions for proper application to prevent access to the foam. CTP will also advise consumers that all US Sirona M seats manufactured after August 31, 2018 come with the additional tape already applied preventing access to the EPS headrest foam and are not the subject of the recall campaign.
3. CTP will put a video on its website demonstrating proper application of the manufacturer-approved tape to the EPS headrest foam. All notification letters sent to consumers will include a QR code to link them to this video as well as a website address where the video is located. Description of the Defect : A small number of consumers notified CTP and NHTSA that is possible for a child to access and pick small pieces of foam under the headrest pad on the US version of the Cybex Sirona M convertible seat. It has been reported that a child can put the small piece of foam into their mouth. These reports have not included any claims of injury or choking. Moreover, the foam picking does not impact the crashworthiness of the seat and the seat is in all other ways compliant with FMVSS 213. However, CTP has determined that a child can access the foam through a small opening in the headrest fabric that covers the foam headrest. It has been reported that a child can pick small pieces of foam which could pose a choking hazard. Pictures of the car seats showing the headrest fabric and headrest foam are included in the CTP response letters to NHTSA dated 7.22.20, 9.20.21 and 12.8.21 in connection with NHTSA inquiry NEF 0102SW/ PE-21-014. The foam pieces that were picked are small (roughly 3-4 mm), generally spherical pieces, with no sharp edges. The foam is non-toxic and does not pose a chemical or poison concern if ingested. CTP closely tracked feedback from consumer owners of the US version of the CYBEX Sirona M following the release of the car seat in the U.S. market. Early in 2018, CTP received a few reports that children could access the foam under the seat pad in the headrest area and could and/or did place the small foam pieces in their mouths. To address consumer feedback and to continuously improve our products for our consumers, CTP implemented an engineering change on August 31, 2018, which consisted of adding tape to the lower edges of the EPS in the child restraintโs headrest. This eliminated the ability of a child from being able to pick or scrape the EPS. NHTSA submitted a request for information to CTP on July 16, 2020 related to two reports of foam picking on the car seat. CTP responded to NHTSA providing information regarding the engineering change. CTP remained in communication with NHTSA regarding the foam picking, providing NHTSA additional requested information on September 20, 2021 and December 8, 2021.
While CTP received some reports of children that could or possibly did place foam pieces in their mouths, CTP did not receive any reports of any injuries or fatalities. The pieces of foam are small, soft, and non-toxic.
However, if a child picks a piece of foam and places it into their mouth, nose, or ear the foam could pose a choking hazard or otherwise cause a safety concern for caregivers. Following additional discussions with NHTSA and in furtherance of continuous product improvement and a robust commitment to safety, CTP determined it should prepare and provide a product improvement kit to consumers through a Voluntary Safety Recall Campaign which will enable consumers who own certain US Sirona M models manufactured on or before August 31, 2018 to apply tape to the EPS foam to prevent foam picking. Description of Remedy Program : CTP plans to send a product improvement kit to owners of the impacted
seats consisting of manufacturer-approved tape and instructions to apply the tape to the EPS headrest foam. The tape will eliminate access to the foam by the occupant. CTP plans to complete the following activities:
1. CTP will mail a notification letter to all registered consumers of the impacted US Sirona M car seats informing them about the potential foam picking issue and enclosing manufacturer-approved tape and instructions for properly applying the tape to prevent access to the headrest foam.
2. CTP will issue a press release on its website and will issue communications on its social media accounts with information about the safety recall campaign and available product improvement kit. Owners will be instructed how to find the model number and date of manufacture on their seat(s). CTP will provide owners of US Sirona M seats manufactured on or before August 31, 2018 who contact CTP with a product improvement kit including manufacturer-approved tape to cover the EPS foam and instructions for proper application to prevent access to the foam. CTP will also advise consumers that all US Sirona M seats manufactured after August 31, 2018 come with the additional tape already applied preventing access to the EPS headrest foam and are not the subject of the recall campaign.
3. CTP will put a video on its website demonstrating proper application of the manufacturer-approved tape to the EPS headrest foam. All notification letters sent to consumers will include a QR code to link them to this video as well as a website address where the video is located. How Remedy Component Differs from Recalled Component : Identify How/When Recall Condition was Corrected in Production : The manufacturer-approved tape will be accompanied by clear written instructions, a QR code linking the consumer to a video demonstrating the proper application of the tape, and a website address where the video instructions can also be found. These instructions will assist the consumer in clearly distinguishing the tape from any existing feature or component of the US Sirona M.
As noted above, based on consumer feedback and as a commitment to continuous product improvement, CTP implemented an engineering change on August 31, 2018, which consisted of adding tape to the headrest EPS foam on the US Sirona M to prevent access to the foam. Recall Schedule :
