Summary
Bendix Commercial Vehicle Systems, LLC (Bendix
) is recalling certain Brake Lights, Model DCM-BB and part number 10049991. The connection between the brake pedal and brake light switch may bind, causing the brake lights to illuminate continuously, or fail to illuminate when the brake is applied.
Remedy
Bendix will work with the vehicle manufacturer to provide replacement part kits, free of charge. Owner notification letters were mailed July 21, 2022. Owners may contact Bendix
customer service at 1-877-345-9526.

) notified the National Highway Traffic Safety Administration (NHTSA) of the start of a voluntary safety recall campaign involving the Bendix
ยฎ Brake Switch Interface within select Bendix
Driver Control Module (DCM)s. The recall has been assigned number 22E033.
A copy of our April 13, 2022, Defect and Noncompliance Information Report submission to NHTSA can be found on the NHTSA website (https://www.nhtsa.gov/recalls) under recall number 22E033. As required by law, Bendix informed NHTSA that Blue Bird is the only vehicle manufacturer customer to whom Bendix
sold the affected products outright.
IMPACTED PRODUCT
This recall includes the brake pedal contained within all Bendix Driver Control Modules (DCM) produced for Blue Bird C-Chassis with hydraulic brakes, a Cummins
ยฎ engine, and a non-adjustable pedal โ brake pedal part number K025587 | Blue Bird DCM part numbers 10049991 and 10049992 โ manufactured between May 1, 2021, and March 16, 2022. This action does not affect DCMs manufactured prior to or after the stated dates, nor those manufactured for any other OEM.
Affected DCMs are identifiable solely by the presence of a serialized, Bendix-generated identification label. Bendix
will provide a list of the affected serial numbers under separate cover.
PROBLEM DESCRIPTION
Bendix has determined that a component manufactured at a Bendix
sub-supplier was not within specification limits and has led to the potential defect โ an undersized hole in the brake pedal that may not allow for proper movement of the brake light switch interface components. The interface between the brake pedal and the brake light switch may bind and not change states, leading to improper brake light operation. Brake lights may not come on when the brake lights are applied, or brake lights may remain illuminated when the brakes are not applied. The binding condition and the associated malfunction may be intermittent.
WHAT WE WILL DO
Bendix Commercial Vehicle Systems LLC (Bendix
) will voluntarily remedy the defect within the DCM with the impacted brake pedal, with a kit of parts that do not have the potential defect. As a reminder, the kit will be provided to you free of charge. The installation labor charge to complete the remedy will be covered by Bendix
.
WHAT YOU NEED TO DO
First, we are providing you with this information to enable your business to determine whether there is a safety-related defect on your vehicles where the affected brake pedal within the Driver Control Module (DCM) has been installed.
Based on your determination, you may need to file a Defect and Noncompliance Report to the National Highway Traffic Safety Administration (NHTSA) with respect to new motor vehicles with an affected Bendix DCM sold in the United States. A similar notification may be required to Transport Canada for all impacted vehicles sold in Canada.

This important voluntary safety recall notice applies to your bus(es) identified by both the Blue Bird Body Number and Vehicle Identification Number (VIN) on the enclosed yellow cover sheet. REASON FOR THIS RECALL

Problem Description
Bendix has determined that a component manufactured at a Bendix
sub-supplier was not within specification limits, leading to the potential defect. The defect is an undersized hole in the non-adjustable brake pedal that may not allow proper movement of the brake light switch interface components. The interface between the brake pedal and the brake light switch may bind and not change states, leading to improper brake light operation. Brake lights may not come on when the brakes are applied, or brake lights may remain illuminated when the brakes are not applied. Theses conditions can increase the risk of a crash.
WHAT WE WILL DO
Bendix Commercial Vehicle Systems, LLC (Bendix
) will voluntarily remedy the defect via a repair procedure and associated parts kit (e.g., the undersized diameter of the hole) on the non-adjustable brake pedal. The required repair kit is currently available and will be provided to you free of charge. This repair will take approximately .05 hours (30 minutes).

Labor Reimbursement
Blue Bird will reimburse the labor cost of the repairs related to this recall at no cost to you the vehicle owner. The Standard Repair Time (SRT) to accomplish this R22BS recall repair โ per vehicle โ is 0.5 hours (30 minutes).
IF YOU HAVE ALREADY REPLACED AN AFFECTED BENDIX BRAKE SWITCH INTERFACE
If the modifications directed by this notification were performed on your bus prior to the receipt of this recall notification, attach a copy of the work order/invoice. Mail the documents in the pink self-addressed postage prepaid envelope included with the pink reply sheet to Blue Bird for warranty consideration. Reimbursements will be made in accordance with the requirements of the National Highway Transportation Safety Act, Title 49 Code of Federal Regulations, Parts 573 and 577.
If you have received this letter and you have replaced an affected non-adjustable brake pedal after March 16, 2022, specifically to address the defect described in this letter; Bendix will reimburse you for your cost of replacement upon Bendix
โs receipt of documentation detailing the replacement unless it was otherwise covered under warranty. If you have claims you believe are directly attributable to this Blue Bird DCM recall, please provide a complete set of documentation to BBswitch22@bendix
.com.

Chronology :
1/27/2022: Bendix engineering received and email from Blue Bird manufacturing that several vehicles had brake lights that were not functioning properly. Other vehicles operated as expected. Joint engineering investigation started.
2/7/2022: Bendix manufacturing added additional testing to confirm that all Driver Control Modules (DCM) left Bendix
with a fully functioning brake light switches.
2/7/2022: Completed vehicles at Blue Bird were inspected for binding brake light switch mechanism.
2/9/2022: Bendix RA (Risk Assessment) team created to evaluate this condition. Investigation included problem solving techniques to determine that only C-chassis, hydraulic braked, non-adjustable pedal vehicle with Cummins
Engines could exhibit this condition.
4/6/2022: The RA team presented its findings to the Bendix Product Integrity Committee (PIC).
4/8/2022 The Bendix Board agreed with the PIC recommendation to report the equipment defect.

Bendix Commercial Vehicle Systems, LLC (Bendix) has decided that a defect which relates to motor vehicle
safety exists in certain Bendixยฎ Brake Switch Interface within select Bendix Driver Control Module (DCM)s.
Bendix notified the National Highway Traffic Safety Administration (NHTSA) of the start of a voluntary safety
recall campaign involving the Bendixยฎ Brake Switch Interface within select Bendix Driver Control Module
(DCM)s on April 13, 2022. The campaign numbers assigned to this recall are noted in the subject line above.
Impacted Product
Bendix is recalling the Bendixยฎ Brake Switch Interface contained within all Bendix Driver Control Modules
(DCM)s produced for Blue Bird C-Chassis with hydraulic brakes, a Cumminsยฎ engine, and a non-adjustable pedal
โ brake pedal part number K025587 | Blue Bird DCM part numbers 10049991 and 10049992 โ manufactured
between May 1, 2021, and March 16, 2022, inclusive. This action does not include the aforementioned Blue Bird
vehicles with Bendix DCMs manufactured outside the indicated manufacture dates, nor those manufactured for
any other OEM.

Problem Description
Bendix has determined that a component manufactured at a Bendix sub-supplier was not within specification
limits, leading to the potential defect. The defect is an undersized hole in the non-adjustable brake pedal that may
not allow proper movement of the brake light switch interface components. The interface between the brake pedal
and the brake light switch may bind and not change states, leading to improper brake light operation. Brake lights
may not come on when the brakes are applied, or brake lights may remain illuminated when the brakes are not
applied. Theses conditions can increase the risk of a crash.
WHAT WE WILL DO
Bendix Commercial Vehicle Systems, LLC (Bendix) will voluntarily remedy the defect via a repair
procedure and associated parts kit (e.g., the undersized diameter of the hole) on the non-adjustable brake
pedal. The required repair kit is currently available and will be provided to you free of charge. This repair
will take approximately .05 hours (30 minutes).

If the modifications directed by this notification were performed on your bus prior to the receipt of this recall
notification, attach a copy of the work order/invoice. Mail the documents in the pink self-addressed postage
prepaid envelope included with the pink reply sheet to Blue Bird for warranty consideration. Reimbursements will
be made in accordance with the requirements of the National Highway Transportation Safety Act, Title 49 Code
of Federal Regulations, Parts 573 and 577.
If you have received this letter and you have replaced an affected non-adjustable brake pedal after March 16,
2022, specifically to address the defect described in this letter; Bendix will reimburse you for your cost of
replacement upon Bendixโs receipt of documentation detailing the replacement unless it was otherwise covered
under warranty. If you have claims you believe are directly attributable to this Blue Bird DCM recall, please

A copy of our April 13, 2022, Defect and Noncompliance Information Report submission to NHTSA can be found
on the NHTSA website (https://www.nhtsa.gov/recalls) under recall number 22E033. As required by law, Bendix
informed NHTSA that Blue Bird is the only vehicle manufacturer customer to whom Bendix sold the affected
products outright.
IMPACTED PRODUCT
This recall includes the brake pedal contained within all Bendix Driver Control Modules (DCM) produced for
Blue Bird C-Chassis with hydraulic brakes, a Cumminsยฎ engine, and a non-adjustable pedal โ brake pedal part
number K025587 | Blue Bird DCM part numbers 10049991 and 10049992 โ manufactured between May 1, 2021,
and March 16, 2022. This action does not affect DCMs manufactured prior to or after the stated dates, nor those
manufactured for any other OEM.
Affected DCMs are identifiable solely by the presence of a serialized, Bendix-generated identification label.
Bendix will provide a list of the affected serial numbers under separate cover.
PROBLEM DESCRIPTION

WHAT WE WILL DO
Bendix Commercial Vehicle Systems LLC (Bendix) will voluntarily remedy the defect within the DCM with
the impacted brake pedal, with a kit of parts that do not have the potential defect. As a reminder, the kit will
be provided to you free of charge. The installation labor charge to complete the remedy will be covered by Bendix.
WHAT YOU NEED TO DO
First, we are providing you with this information to enable your business to determine whether there is a safetyrelated
defect on your vehicles where the affected brake pedal within the Driver Control Module (DCM) has been
installed.
Based on your determination, you may need to file a Defect and Noncompliance Report to the National
Highway Traffic Safety Administration (NHTSA) with respect to new motor vehicles with an affected
Bendix DCM sold in the United States. A similar notification may be required to Transport Canada for all
impacted vehicles sold in Canada.

Description of Recall Schedule : Blue Bird is the only customer for this equipment. No sales were made
from Bendix to a vehicle owner or vehicle or equipment dealer. Blue Bird
was notified by email and in writing on May 26, 2022. Since Blue Bird is
the only customer, Blue Bird notified Bendix in a teleconference, that
Blue Bird would not perform a vehicle recall. Bendix will receive a list of
Blue Bird dealerships and Vehicle owners. Bendix, with tactical support
from Blue Bird, will notify Blue Bird dealers and Owners on or before
Aug 1, 2022.
