For Canadian owners, this notice is sent to you in accordance with the requirements of the Motor Vehicle Safety Act. This is to inform you that your vehicle may contain a defect that could affect the safety of a person. Altec Industries, Inc. has decided that a defect which relates to motor vehicle safety exists in AT200A aerial devices built on cutaway vans from January 2016 to January 2022. These units have a GFCI outlet that could become overloaded during use. Overloading of a GFCI outlet could cause it to fail and produce a fire. Death or serious injury could result from a fire. Refer to the included notice for the items covered under the Altec Warranty Policy. If you had this repair performed before you received this notice, you may be eligible to receive reimbursement for the cost of obtaining a pre-notification remedy of the problem associated with this recall. All work will be performed at no charge to the customer when presented for repair. Compare your unit’s identifying information with the provided list to verify your unit is affected. You may also contact Altec or view your fleet through Altec Connect to determine if there are any other outstanding notices. The inspection and repair can be performed by the customer, or you may contact Altec for further assistance. The inspection is expected to take 15 minutes to complete. The repair is expected to take 2 hours to complete. Parts may not be immediately available due to supplier backorder. If you have sold or retired the unit, update the records through Altec Connect. Verify your unit is affected by reviewing the attached list or accessing Altec Connect. Background: Altec has learned that the affected units have a generator with a maximum current output that exceeds the 20 amp rating of the GFCI outlet(s) connected to it. A GFCI outlet could be overloaded by extended use at a load exceeding 20 amps. This can happen when simultaneously running the air conditioning and using a tool plugged into the outlet. WARNING Death or serious injury could result from a fire. Overloading of a GFCI outlet could cause it to fail and produce a fire. Customer Action: Do not use the GFCI outlet(s) in the enclosed work area until the new circuit breaker(s) is installed. Until then, only use the power strip that is powered by the inverter. This power strip is usually located on the left side of the work bench that is mounted on the curb side wall of the enclosed work area. Inspect the unit as described in the Inspection Procedure beginning on page 2. Order and install the proper Circuit Breaker Kit as based on the inspection, or contact Altec to perform this installation. The kit must be installed no later than the next preventive maintenance interval or 90 days from the receipt of this CSN, whichever comes first. Requirements: Every unit requires the installation of the proper kit for completion. The inspection is estimated to take 15 minutes and one person to complete. The kit installation is estimated to take 2 hours and 1 person to complete. Completion and Warranty: The inspection and repair are covered under the Altec Warranty Policy and can be performed by Altec, the customer, or the customer’s warranty provider. Altec will perform the work for free at an Altec facility. If the customer or the customer’s warranty provider performs the work, a warranty claim must be submitted to be reimbursed for the cost of the parts and/or labor. Altec will allow up to $22.50 for the labor to perform the inspection and up to $180 for the labor to perform the repair. Customers are responsible for the travel costs of an Altec Mobile Service technician if the technician performs the work at the owner’s location. Read and understand all steps of the instructions before beginning the procedure. 1. Position the unit on a level surface, and apply the parking brake. Turn off the engine, and chock the wheels. 2. Enter the enclosed rear work area of the van through one of the roll-up doors. Find the GFCI electrical outlet box located on the street side of the rear wall next to the rear roll-up door. 3. Determine the required Circuit Breaker Kit part number, which depends on whether the outlet box contains one or two GFCI duplex outlets (refer to Figures 1 through 3). • One GFCI duplex outlet—kit part number 991524604 • Two GFCI duplex outlets—kit part number 991524605 4. Order the kit specified in step 3 and install it upon receipt, or contact Altec and schedule to have the kit specified in step 3 installed. 5. Apply masking tape or duct tape over the front of the GFCI outlet box. Write “DO NOT USE PER CSN-3008” on the tape with a permanent marker. 6. Put the unit back into service while awaiting the kit installation. Contexte : Altec a appris que les unités concernées sont équipées d’un générateur dont le courant de sortie maximal dépasse la limite de 20 ampères de la ou des sorties protégées par DDFT qui y sont connectées. Une sortie protégée par DDFT pourrait être surchargée durant une utilisation prolongée avec une charge dépassant 20 ampères ce qui peut survenir en cas d’utilisation simultanée de la climatisation et d’un outil branché dans la prise. AVERTISSEMENT Un incendie peut entraîner la mort ou des blessures graves. Toute surcharge d’une sortie protégée par DDFT pourrait entraîner sa défaillance et un incendie. Action requise de la part du client : N’utilisez pas la ou les sorties protégées par DDFT dans la cabine de travail avant que le ou les coupe-circuits ne soient installés. Jusqu’à ce que ce soit fait, utilisez seulement la barre multiprise alimentée par l’onduleur. Cette barre se trouve généralement sur le côté gauche de l’établi fixé sur la paroi latérale du côté trottoir de la cabine de travail. Inspectez l’unité de la manière indiquée au début de la marche à suivre de l’inspection en page 2. Commandez et installez le kit de coupe-circuits correct en fonction du résultat de l’inspection ou contactez Altec qui se chargera de cette installation. Le kit doit être installé au plus tard lors du prochain intervalle de maintenance préventive ou dans les 90 jours suivant la réception de ce CSN, selon la première éventualité. Electrical components could be connected to the receptacle that could draw up to 30-amp through the outlet causing overload and potential failure. Address : 2106 S Riverside Rd Saint Joseph Missouri 64507-2535 Country : United States Chronology : On February 2, 2022, a customer notified Altec that a service technician had identified that the generator circuit on an AT200A unit was rated at 30-amps with a connected 20-amp GFCI receptacle. The GFCI receptacle circuit does not have a circuit breaker installed to open the circuit when an overload of the circuit is detected. After being notified, Altec reviewed production records. On May 3, 2022, a potential field action was opened to further evaluate customer work practice, the risk to workers, and determine what Models were affected and the field population of those units. On June 24, 2022, Altec determined that a potential defect was present in a certain field population and determined that a field campaign was required. There have been no warranty claims or other field reports identified. There have been no accidents, injuries, or deaths related to this issue. Description of Remedy : Description of Remedy Program : A new outlet will be installed with a circuit-breaker and new wiring. The recall remedy will be completed free of charge when presented for repair. The Altec Warranty Policy covers the recall remedy including cases requiring reimbursement for the cost of obtaining a pre-notification remedy of the problem associated with this recall.
