Insufficiently Welded Stokes Basket Brackets
The left and/or right brackets can fail, allowing the stokes basket to fall and increasing the risk of injury.

Pierce Manufacturing (Pierce
) is recalling certain 2010-2022 Pierce
Velocity, Enforcer, Arrow XT, Quantum, and Dash CF fire trucks. Stokes basket mounting brackets may be insufficiently welded.
Remedy
Pierce will replace the stokes basket mounting brackets, free of charge. Owner notification letters were mailed December 14, 2022. Owners may contact Pierce
’s customer service at 1-888-974-3723. Pierce
’s number for this recall is 74B323.

Pierce has issued a safety campaign on the stokes basket LYFE brackets used on the RMAP and 100’ mid-mount tower. The customer notifications will begin mailing this week while replacement parts are in process and will be available for shipment thereafter.
Execution Details:
- Dealers will place an order for one (1) each of part numbers 4247753 and 4247754 per unit.
- Dealers will transfer the tether hardware from the old brackets to the replacement brackets. o Drill an 11/64” pilot hole in the new bracket. Use the old bracket as reference for location. o Detach tether hardware from old bracket and attach to the new bracket with a 10-24 x ½” screw Dealer will render the old brackets unusable and photograph destruction and disposal of recalled product.
- Dealer will submit campaign claim to Pierce
with photo attached.
Reimbursement:
- Labor – One half (0.5) hour to replace the two stokes baskets LYFE brackets with transfer of hardware. § Material – Cost of the brackets; including freight. § Travel – Travel will not be reimbursed.
NOTE: Failure to include evidence of old brackets being rendered unusable will result in claim rejection.

Chronology :
October 13, 2022 – Pierce was notified by a fire department that during an event which included the placement of a stokes basket onto the stokes basket mounting brackets, a failure occurred which permitted the stokes basket to become unsecured to the stokes basket mounting brackets. No injuries were reported.
October 14, 2022 – Pierce convenes a team comprised of engineering, quality, and manufacturing resources to assess the issue.
October 14, 2022 – Pierce evaluates the performance of three stokes basket mounting brackets in-house. Pierce
observes that weld penetration on two of these three brackets do not meet manufacturing criteria. Pierce
takes steps to quarantine all vehicles that contain the stokes basket mounting bracket option at the manufacturing facility.
October 25, 2022 – Pierce decides to conduct a safety recall to address the issues involving the welds on the stokes basket mounting brackets. Pierce
is not aware of any injuries or warranty claims allegedly related to this issue, however, in an abundance of caution, Pierce
intends to contact owners of all trucks which the stokes basket mounting bracket option shipped with and recall all left hand and right hand stokes basket mounting brackets since the start of production.

A pair of these brackets (left hand and right hand) can be mounted to the platform of an aerial truck and are used to secure a stokes basket, a device which is intended to hold and transport a victim. In some cases, the welds on the brackets which hold the stokes basket may not have been sufficiently welded, potentially resulting in a failure of the brackets. Stokes basket mounting brackets are only available as an option on 100 Foot Rear Mount Aluminum Platforms (RMAP’s) and 100 Foot Mid Mount Steel Platforms of the vehicle models referenced above. A pair of these brackets (left hand and right hand) can be mounted to the platform of an aerial truck and are used to secure a stokes basket, a device which is intended to hold and transport a victim. In some cases, the welds on the brackets which hold the stokes basket may not have been sufficiently welded, potentially resulting in a failure of the brackets. Stokes basket mounting brackets are only available as an option on 100 Foot Rear Mount Aluminum Platforms (RMAP’s) and 100 Foot Mid Mount Steel Platforms of the vehicle models referenced above. Insufficient weld penetration could cause components of the brackets to fail. No injuries were reported. October 14, 2022 – Pierce convenes a team comprised of engineering, quality, and manufacturing resources to assess the issue. October 14, 2022 – Pierce evaluates the performance of three stokes basket mounting brackets in-house. Pierce observes that weld penetration on two of these three brackets do not meet manufacturing criteria. Pierce takes steps to quarantine all vehicles that contain the stokes basket mounting bracket option at the manufacturing facility. October 25, 2022 – Pierce decides to conduct a safety recall to address the issues involving the welds on the stokes basket mounting brackets. Pierce is not aware of any injuries or warranty claims allegedly related to this issue, however, in an abundance of caution, Pierce intends to contact owners of all trucks which the stokes basket mounting bracket option shipped with and recall all left hand and right hand stokes basket mounting Part 573 Safety Recall Report 22V-809 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 brackets since the start of production. Description of Remedy : Description of Remedy Program : Pierce will notify current owners who have the stokes basket mounting bracket option with their truck to remove the brackets from service and send them back to Pierce. Pierce will supply new left hand and right hand stokes basket mounting brackets at no charge to existing customers. How Remedy Component Differs from Recalled Component : The remedied component will incorporate dimensional changes which will permit (1) better placement in fixturing and (2) robotic welding for the affected pieces where the weld failures were observed. Identify How/When Recall Condition was Corrected in Production : Pierce, upon the initial notice of the stokes basket bracket mounting failure, immediately quarantined all stokes basket mounting brackets in process. Pierce continues to deliver trucks however Pierce is not delivering any stokes basket mounting brackets with trucks until the defined remedy is in place. Recall Schedule : Description of Recall Schedule : Notifications to customers will be made via a customer letter. The 577 customer letter will be mailed pending NHTSA approval.
