Summary
Motor Coach Industries (MCI
) is recalling certain 2019-2022 D4000 ISTV, D4500, 2021 D4000, D4520, 2020-2021 D4505, D4005, 2020-2022 D45CRTLE, and 2019-2021 J4500 vehicles equipped with certain Cummins
X12 diesel engines. The fuel tubes between the fuel rail and the injectors for cylinders four, five, and six may fatigue and crack, which can result in a high pressure fuel leak.
Remedy
On engines with 75 miles or less, Cummins dealers will install vibration isolators to the fuel tubes. Engines with more than 75 miles will receive new fuel tubes with vibration isolators. Repairs will be performed free of charge. Owner notification letters were mailed March 11, 2022. Owners may contact MCI

On 6 December 2021 Cummins Inc. advised Motor Coach Industries
(MCI
) that Equipment Recall 21E-099 had been declared with NHTSA. Cummins
provided a listing of affected Engine Serial Numbers (ESN) sold to MCI
. MCI
began cross-matching ESN’s to Vehicle Identification Numbers.
On 8 December 2021 the MCI Vehicle Safety Committee determined that a vehicle recall would be declared with NHTSA and Transport Canada.
On 15 December 2021 MCI declared a Vehicle Recall with NHTSA.

Cummins has released field Safety Campaign #C2532, X12 CM2350 X119B and X12 CM2450 X137B Fuel Injector Supply Line Safety Campaign to correct this condition by replacing the fuel tubes with new fuel tubes and installing vibration isolators for cylinders 4,5 and 6. This campaign is currently available at Cummins Distributors and authorized Warranty dealers. The repair will be completed free of charge and most applications will require approximately 2 hours to complete. WHAT SHOULD YOU DO? Contact the nearest Cummins Distributor or authorized Warranty Dealer to arrange to have this campaign performed on your engine. The distributor or dealer will work with you to schedule the best date to complete the repair. For all other engines, the subject fuel tubes for cylinders 4, 5 and 6 (the rear half of the engine) will be replaced with new fuel tubes (having the same part number as the recalled components) and vibration isolators will be added to the fuel tubes to engines. Cummins Inc. is paying parts and labor to accomplish this corrective action. Once parts are available, customers will receive written notification from Cummins Inc. decribing how to get this corrective action incorporated on thier vehicles at no cost by any authroized Cummins dealer. How Remedy Component Differs from Recalled Component : The replacement fuel tubes will also have vibration isolators. The fuel tube and vibration isolator combination will have a new part number. On 8 December 2021 the MCI Vehicle Safety Committee determined that a vehicle recall would be declared with NHTSA and Transport Canada. On 15 December 2021 MCI declared a Vehilce Recall with NHTSA. Description of Remedy : Description of Remedy Program : For engines having 75 miles or less, the remedy will consist of adding vibration isolators to the fuel tubes; fuel tubes do not need to be replaced. For all other engines, the subject fuel tubes for cylinders 4, 5 and 6 (the rear half of the engine) will be replaced with new fuel tubes (having the same part number as the recalled components) and vibration isolators will be added to the fuel tubes to engines. Cummins Inc. is paying parts and labor to accomplish this corrective action. How Remedy Component Differs from Recalled Component : The replacement fuel tubes will also have vibration isolators. The fuel tube and vibration isolator combination will have a new part number. Identify How/When Recall Condition was Corrected in Production : Cummins has identified the first engine shipped to MCI which has the corrective action implemented as ESN 75099185. All 2017-2021 MY Cummins X12 engine recieved prior to that will be subject to corrective action prior to shipping/installation.
