All units produced in the dates identified above and sold by Harbor Freight share the same pressure relief plug design, which was unchanged throughout the life of the product. Production Dates : AUG 19, 2019 – NOV 18, 2022 Description of Defect : Description of the Defect : For certain units of the Holt Deluxe Manual Fluid Extractor and Dispenser (SKU 56384), a pressure relief plug may not operate properly under certain circumstances when the product is used in dispense mode and there is a restriction or blockage in the outlet tube. In the event the outlet becomes blocked, the pressure relief plug may not activate to relieve the pressure within the unit in certain units when in dispense mode. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : If pressure is unable to be released via the pressure relief plug, with continued use in dispense mode, pressure could eventually build enough to cause lid of the unit to unexpectedly separate from the body of the unit, increasing the risk of injury to users. Description of the Cause : NR Part 573 Safety Recall Report 23E-006 Page 2 The information contained in this report was submitted pursuant to 49 CFR §573 Identification of Any Warning that can Occur : If the product is over-pressurized while in dispense mode, the lid may bulge and/or the pressure relief plug may begin to dislodge prior to the top of the unit separating from the body of the unit. Address : No. 753, Sec 1, Si-Nan Rd., Wurih, 41465 Taichung City Foreign States Country : Taiwan, Province of China Chronology : As a result of a customer email alleging that the Holt Deluxe Manual Fluid Extractor and Dispenser (SKU 56384) malfunctioned during use (but caused no injury), HFT engineers conducted an internal investigation, including the review of additional reports alleging similar malfunctions of the same product, some alleging injuries to users. HFT performed pressure relief tests with fluids of varying viscosities using new product from its inventory. The investigation showed that the majority of the samples in HFT’s tests worked as intended, but the pressure relief device in the product may not consistently operate as intended if the fluid flow out of the product is restricted while in the dispense mode (e.g., the hose is kinked or contamination is blocking the outlet of the hose). On December 8, 2022, HFT implemented a stop sale to prevent any further sales of the product. On December 22, 2022, based on the multiple applications for which the product could be used (including multiple uses outside of automotive applications), HFT submitted its Initial Report to the Consumer Product Safety Commission (CPSC) under its Fast Track Recall Program. On January 4, 2023, HFT sent an additional reminder to its distribution chain that the products are subject to a stop sale. On January 23, 2023, HFT submitted its full report to the CPSC. Part 573 Safety Recall Report 23E-006 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 On January 30, 3023, CPSC informed HFT that the product “do[es] not fall within the matter jurisdiction of the Commission. Please forward all future correspondence on this matter to: NHTSA.” This letter precipitated this submission to NHTSA. HFT intends to submit a copy of CPSC’s letter to NHTSA. On January 31, 2023, HFT decided to submit a defect information report to NHTSA for this product consistent with CPSC’s instructions. Description of Remedy : Description of Remedy Program : The remedy will be a component part replacement of the product lid that the user can easily install. Specifically, HFT will send the lid ring and pour spout cap with a new integrated pressure relief valve to the consumer with instructions on how to replace the part at issue in this recall. HFT has the name and contact information for over 91% of customers who purchased the Deluxe Manual Fluid Extractor and Dispenser (SKU 56384). HFT will directly contact all consumers for whom it has contact information to confirm current mailing addresses to provide replacement assemblies/repair kits. Further, HFT will publicize the Recall Notice through outlets it controls—its website, social media accounts, and stores. How Remedy Component Differs from Recalled Component : Whereas the recalled components have a pressure relief plug integrated into the vacuum line and a less rigid screw cap (blue in color), the remedy components will have (1) a replacement spout cap that contains a new integrated pressure relief valve that is designed to activate at approximately 20 psi, and (2) a replacement screw cap constructed of more rigid material that resists higher pressure. In the event that the user is operating in the dispense mode, and fluid flow out of the product is restricted, the additional pressure relief valve as stated in Remediation Step 1 will activate when the pressure in the tank reaches approximately 20 psi, effectively and timely reducing the pressure in the tank. The replacement screw cap constructed of a more rigid material as stated in Remediation Step 2 will also reduce the potential for a user overpressurizing the product. Consumers will be advised to dispose of the original spout and screw cap. The recalled components do not have any specific markings. However, the replacement screw cap will be black (instead of a blue screw cap in the existing product). Additionally, the replacement spout cap will have the new integrated pressure relief valve to distinguish it from the existing product. New products in Harbor Freight’s inventory will be repaired with the replacement components (i.e., new replacement spout cap with integrated pressure relief valve and black replacement screw cap) and the product will be designated with a new UPC code to reflect the repair. On January 4, 2023, HFT sent an additional reminder to its distribution chain that the products are subject to a stop sale. Further, HFT stopped any future orders from the factory until the updated screw cap and replacement spout cap are installed on every product.
