Production Dates : SEP 12, 2017 – NOV 02, 2022 VIN Range 1 : Begin : NR End : NR Not sequential Description of Defect : Description of the Defect : Two bolts and nuts that secure the removable hex extension bar to the drive unit coupler were not installed on some assemblies. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : With the digger drive unit deployed, if the removable hex extension bar detached from the drive unit coupler, an auger or other attachment could fall, causing a risk of injury to personnel near the unit. Description of the Cause : NR Identification of Any Warning that can Occur : A preoperation inspection could identify that the hardware is missing. Address : 701 East Joppa Road-MR135 Towson Maryland 21286 Country : United States Chronology : On or around February 6, 2023, a customer notified Altec that the auger extension bar assembly was missing the bolts that secure the extension bar to the drive unit coupler on one of their units. On February 14, 2023, Altec contacted the supplier, Stanley Black & Decker, Inc. (SBD), to order the repair kit and inform them of the issue. On February 27, 2023, Altec sent a Supplier Corrective Action Request to SBD to document and communicate the details of the issue. On March 3, 2023, an investigation was initiated to determine if units in the field could have an auger extension bar assembly missing hardware, the detectability of missing hardware, and the safety risk of missing hardware. On April 28, 2023, after reviewing Altec’s and SBD’s investigative results, Altec determined that a defect could exist on digger derrick units equipped with the affected auger extension bar assemblies. There were no other warranty claims or field reports identified. There have been no accidents, injuries, or deaths related to this issue. Part 573 Safety Recall Report 23V-326 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 Description of Remedy : Description of Remedy Program : Altec will issue CSN 3076 to affected unit owners requiring owners to inspect the auger extension assembly for missing hardware. If the hardware is missing, the needed replacement kit will be provided by Altec free of charge. The Altec Warranty Policy covers the recall remedy, including cases requiring reimbursement for the cost of obtaining a prenotification remedy of the problem associated with this recall. How Remedy Component Differs from Recalled Component : The remedy kit includes the hardware to secure the auger extension bar to the input drive coupler. Identify How/When Recall Condition was Corrected in Production : The hardware is being installed and inspected by the supplier, and Altec is completing a receiving inspection of the assemblies.
