Chronology :
In December 2022, a customer reported that a strobe light on the unit had caused a thermal event that caused damage to the unit body and other components around the strobe light. Altec investigated the incident and found an electrical arc witness mark and moisture inside the lens. The subject strobe light was returned to the manufacturer for evaluation.
On February 6, 2023, a second customer notified Altec of a similar thermal event involving the same strobe light part number in the December incident. Around March 10, 2023, an additional customer reported a similar incident. All subject strobe lights were returned to the supplier for evaluation.
On March 14, 2023, Altec opened a potential field action investigation and worked with the supplier to determine the root cause of the light failures and the potential affected population of strobe lights installed on Altec units. On May 12, 2023, after reviewing Altec’s and TecNiq’s investigation results, Altec determined that a defect could exist on Altec units with the subject part number strobe light installed. There were no other warranty claims or field reports identified. There have been no accidents, injuries, or deaths related to this issue.
