Description of the Cause : The red Reflex Reflectors located at the rear of the chassis were not installed during production as required per FMVSS 108 requirements. Identification of Any Warning that can Occur : NR Involved Components : Component Name 1 : NR Component Description : NR Component Part Number : NR Supplier Identification : Component Manufacturer Name : NR Address : NR NR Country : NR Chronology : During a Sales Call in April involving a walk-around of a recent production 40 ft Chassis with a potential new Customer, it was brought to Fontaine’s attention that the unit may not be in compliance with FMVSS 108 Reflector requirements. Subsequent review of “like” units located at the Fontaine facility confirmed that the red Reflex Reflectors on the rear corners had been omitted during production of the units. Description of Remedy : Description of Remedy Program : Fontaine will supply all affected customers with 4 red Reflex Reflectors per unit to correct the units in their possession. Fontaine will cover all costs associated with making the vehicles compliant per FMVSS 108 requirements. How Remedy Component Differs from Recalled Component : The remedy will involve installing red Reflex Reflectors that were omitted during production. Fontaine has inventory on hand to service the affected units.
