Production Dates : AUG 01, 1999 – OCT 31, 2022 VIN Range 1 : Begin : NR End : NR Not sequential Description of Defect : Description of the Defect : Jib and Platform positioning cylinders on certain Altec aerial devices could have a hydraulic hose installed between a cylinder port and the load-holding valve. Hose failure could allow motion of the platform or material handling jib. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : Damage to the hydraulic hose could cause a failure of the load-holding feature and allow the platform or load to have uncontrolled movement increasing the risk of injury or death to the operator or others nearby. Description of the Cause : NR Identification of Any Warning that can Occur : When the inspector knows the tubing is required, the hose can be identified in a periodic inspection. If the hose is damaged, a nick or other witness mark or oil leak might be visible before uncontrolled motion. Address : 2106 S Riverside Rd Saint Joseph Missouri 64507-2535 Country : United States Chronology : In 2021, Altec identified units in production with hydraulic hoses installed in locations where metallic tubing was specified. A production change was made to improve the manufacturability of the assemblies. In 2022 production continued to report difficulty with the assemblies. On September 8, 2022, a potential field action investigation was opened to determine if improper assemblies had reached the field. The investigation resulted in service instructions being launched on November 8, 2022, to collect data on the units in the field to determine the potential affected population. On May 16, 2023, the data collection work was closed after the required level of information was collected. The collected data was reviewed to determine affected units’ assemblies, unit models, and production dates. On June 9, 2023, Altec determined a defect was present on a specific population of aerials that might not comply with aerial device standards and a field campaign was required. There were no warranty claims or field reports identified. There have been no accidents, injuries or deaths related to this issue. Description of Remedy : Description of Remedy Program : The affected units will be inspected for a hydraulic hose installed in an incorrect location. If a hose is present, it will be replaced with the specified metallic tubing. The recall remedy will be completed free of charge when presented for repair. The Altec Warranty Policy covers the recall remedy including cases requiring reimbursement for the cost of obtaining a prenotification remedy of the problem associated with this recall. How Remedy Component Differs from Recalled Component : The remedy will use metallic tubing between the cylinder port and the load-holding device. Identify How/When Recall Condition was Corrected in Production : Production updated the work instructions, training, and component stocking in October of 2022.
