Fuel Filter Hoses May Disconnect or Leak Fuel

Diesel Engines part number, D563027BX03. The remote Stage 2 fuel filter hoses may have been damaged during production, allowing the hoses to leak or disconnect.
Remedy
Dealers will replace both hoses, free of charge. Owner notification letters are expected to be mailed September 15, 2023. Owners may contact Cummins

We are writing to inform you that Cummins
Inc. has determined that a safety-related defect exists in certain Cummins
L9
engines equipped with a remote Stage 2 fuel filter (Option Number FF90942), which includes two pressurized fuel hoses that were manufactured between November 22, 2022 and February 28, 2023. Cummins
has initiated a recall of the subject L9
engines. Our records indicate that some of these engines were shipped to you.
A copy of Cummins
’ initial Part 573 Defect Notification report is enclosed. This notification was submitted to the National Highway Traffic Safety Administration (NHTSA) on July 18, 2023. As more fully described in the Defect Notification, the remote fuel filter hoses installed on some of the subject engines may have been damaged during engine production, possibly resulting in a fuel hose becoming disconnected that will result in an engine stall or a fuel leak. A stall may increase the risk of a crash. A fuel leak in the presence of an ignition source may increase the risk of a fire.
Enclosed is a list of serial numbers for affected engines that our records indicate were shipped to your plants. For engines that have left your control, we encourage you to make a safety defect determination in accordance with 49 USC 30118 et seq. and, as appropriate, to submit a Defect Notification to NHTSA in accordance with 49 CFR Part 573. Cummins
will conduct the recall, including notifying owners pursuant to 49 CFR Part 577, performing the remedy, and submitting quarterly status reports to NHTSA. Cummins
is requesting that you provide the associated VINs and owner contact information to facilitate owner notification. Please work with your Cummins
account team leader to ensure that the requested information is provided to Cummins
as soon as possible.
As an equipment manufacturer, Cummins
has no obligation to file a defect notification to Transport Canada for this recall. If an OEM has an obligation to report the defect to Transport Canada, we ask that you please provide the Transport Canada recall number for your vehicle recall to Cummins
. Cummins
cannot mail owner letters compliant with Transport Canada regulations without the recall number assigned by Transport Canada. Cummins
will provide information to the OEM after mailings are completed so that the OEM can submit the required reports to Transport Canada.
For engines that are still in your inventory, please contact the Owner Quality Assurance representative at the Rocky Mount Engine Plant to arrange for repairs. Please note that Federal law prohibits delivery of these engines, or vehicles equipped with these engines, under a sale or lease until this condition has been remedied.

is finalizing the plan to make a field service safety campaign available to address this condition and will advise you when the schedule is known.

Chronology :
1/12/23–Cummins
was informed of leaks on the subject hoses during assembly at a PACCAR
production facility.
1/12/23–3/3/23–Cummins
completed an investigationinto the cause of the leaks. Based upon a preliminary fault tree analysis that identified insufficient crimping as the most likely cause of leaks, on 3/3/23 Cummins
implemented hoses with an improved connector crimp to increase the robustness of the hoses.
3/3/23–4/28/23–Cummins
monitored the improvement. An additional leak with the improved hoses was reported to Cummins
on 4/28/23 by an OEM customer that observed the leak during their assembly process.
4/28/23–5/10/23–Cummins
continued to investigate the issue and determined that the cause was that the hoses may have snagged on the assembly pallet. 5/14/23–Cummins
implemented an additional corrective action to prevent the snagging by controlling the hose routing during production and implemented a check for damaged hoses.
4/17/23-6/13/23–Cummins
conducted a product safety investigation. A warranty search identified 52 claims with claim dates of 1/9/23-6/13/23 for leaks involving engines equipped with this option. Cummins
confirmed that there had not been any field reports of leaks involving engines built after 3/3/23 when the improved hoses were implemented. Cummins
also concluded that the improved hoses are sufficient to address the issue even if the improved hoses may have snagged on the pallet prior to 5/14/23 when the snagging corrective action was implemented. Cummins
conducted a Product Safety Hazard Analysis to evaluate the potential safety risk on fielded units.
6/14/23-7/11/23 – Cummins
escalated the issue through its Product Safety Defect Determination Process.
7/11/23 – Based upon the results of the investigation, the Cummins
Product Safety Defect Board decided to conduct a safety campaign to address this condition. To date, there have been no reports of fires, accidents or injuries related to this issue.

The recall consists of the entire population of the engines built between the identified production dates that were equipped with this remote fuel filter option. The subject engines were sold to various OEMs for installation on trucks and Recreational Vehicles. The production “Begin” and “End” dates provided herein are based on the dates listed on the affected engines’ dataplates (the engine “build date”). Production Dates : NOV 22, 2022 – FEB 28, 2023 Description of Defect : Description of the Defect : The remote fuel filter hoses installed on some of the subject engines may have been damaged during engine production, possibly resulting in a fuel hose becoming disconnected or leaking fuel. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : A fuel hose that becomes disconnected will result in an engine stall, increasing the risk of a crash. engines equipped with a remote Stage 2 fuel filter (Option Number FF90942), which includes two pressurized fuel hoses. The recall consists of the entire population of the engines built between the identified production dates that were equipped with this remote fuel filter option. The subject engines were sold to various OEMs for installation on trucks and Recreational Vehicles. The production “Begin” and “End” dates provided herein are based on the dates listed on the affected engines’ dataplates (the engine “build date”). Production Dates : NOV 22, 2022 – FEB 28, 2023 Description of Defect : Description of the Defect : The remote fuel filter hoses installed on some of the subject engines may have been damaged during engine production, possibly resulting in a fuel hose becoming disconnected or leaking fuel. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : A fuel hose that becomes disconnected will result in an engine stall, increasing the risk of a crash. A fuel leak in the presence of an ignition source may increase the risk of a fire. The damage may have gone unnoticed. Identification of Any Warning that can Occur : Persons operating in or around the vehicle may see smoke or other visible signs of a fuel leak. Persons operating in or around the vehicle may also smell diesel fuel. production facility. 1/12/23–3/3/23–Cummins completed an investigationinto the cause of the leaks. Based upon a preliminary fault tree analysis that identified insufficient crimping as the most likely cause of leaks, on 3/3/23 Cummins implemented hoses with an improved connector crimp to increase the robustness of the hoses. 3/3/23–4/28/23–Cummins monitored the improvement. An additional leak with the improved hoses was reported to Cummins on 4/28/23 by an OEM customer that observed the leak during their assembly process. 4/28/23–5/10/23–Cummins continued to investigate the issue and determined that the cause was that the hoses may have snagged on the assembly pallet. 5/14/23–Cummins implemented an additional corrective action to prevent the snagging by controlling the hose routing during production and implemented a check for damaged hoses. 4/17/23-6/13/23–Cummins conducted a product safety investigation. A warranty search identified 52 claims with claim dates of 1/9/23-6/13/23 for leaks involving engines equipped with this option. Cummins confirmed that there had not been any field reports of leaks involving engines built after 3/3/23 when the improved hoses were implemented. Cummins also concluded that the improved hoses are sufficient to address the issue even if the improved hoses may have snagged on the pallet prior to 5/14/23 when the snagging corrective action was implemented. Cummins conducted a Product Safety Hazard Analysis to evaluate the potential safety risk on fielded units. 6/14/23-7/11/23 – Cummins escalated the issue through its Product Safety Defect Determination Process. 7/11/23 – Based upon the results of the investigation, the Cummins Part 573 Safety Recall Report 23E-055 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 Product Safety Defect Board decided to conduct a safety campaign to address this condition. To date, there have been no reports of fires, accidents or injuries related to this issue. Description of Remedy : Description of Remedy Program : The remedy is to replace the two subject fuel hoses with hoses having improved connector crimps. Cummins requests that it be exempt from providing notification of a reimbursement plan. Any pre-notification product failure would have been corrected under the manufacturer’s limited warranty. Accordingly, no person would be eligible for reimbursement pursuant to 573.13. How Remedy Component Differs from Recalled Component : The remedy component has an improved crimp and different date codes. Remedy hoses will have a date code of 230310 (YYMMDD format) and later. Identify How/When Recall Condition was Corrected in Production : The condition was corrected in production on 3/3/23 with the introduction of an improved hose design with an increased crimp on the connector joint. Recall Schedule : Description of Recall Schedule : Cummins expects to notify the affected OEMs by 07/28/2023. Pending consultation and agreement with the affected OEMs, Cummins will conduct the recall, including sending owner notifications. The timing of owner notification will be determined in consultation with the OEMs. Cummins expects to release a safety campaign by September 7, 2023.
