Production Dates : JAN 16, 2022 – AUG 07, 2023 Description of Defect : Description of the Defect : Due to a deviation from the design standard, the fabric closure on the outboard panel of the seat cover does not include the necessary low tensile (Tek-40) thread to accommodate the deployment of a side-seat installed airbag in the event of a qualifying impact. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : It is possible that the air bag inflation could be impacted (i.e. slow inflation, lack of inflation, delayed inflation) by the seat cover. Description of the Cause : During physical patterning of the seat cover, the side airbags on the applicable seat were not identified (no sewn-in SRS tag) during inspection. On a subsequent patterning visit to add additional products for this make/model, Part 573 Safety Recall Report 23E-066 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 the “SRS” indicator was identified stamped into the plastic molding at the base of the seat. This alerted the design team to the nonconformance to design standards. Identification of Any Warning that can Occur : None Involved Components : Component Name : Backrest portion of seat cover Component Description : NR Component Part Number : NR Supplier Identification : Component Manufacturer Name : NR Address : NR NR Country : NR Chronology : On or around August 9, 2023, Design employees performing design patterning on a 2023 Toyota Sienna rear captain chair, identified a stamped “SRS” at the base of the rear seat indicating the seat was equipped with a side seat (outboard) airbag. The designer was aware that the existing design for the seat cover did not include a required low-tensile thread (low tensile thread to allow for effective deployment of the airbag). On or around August 9, 2023, the design employee notified Design Management of the concern that a low tensile thread should be included on this product to accommodate the outboard airbag. Design management immediately notified executive leadership. Executive leadership requested that the Product Recall Team (PRT) be convened on August 21, 2023. Full quarantine (part numbers systemically locked in sale system) implemented for applicable Toyota Sienna part numbers on August 10, 2023. On August 21, 2023, the PRT convened and determined that additional investigation was required. On August 22, 2023, design staff completed their investigation and determined that the seat cover could possibly impact the effective deployment of the airbag. On August 24, 2023, the Chief Operations Officer conferred with legal counsel and determined that a potential Part 573 Safety Recall Report 23E-066 Page 4 The information contained in this report was submitted pursuant to 49 CFR §573 recall submission to NHTSA would be required. On August 24, 2023, an additional audit was performed of existing seat cover patterns and two additional models, 2018-2022 Ford Ecosport and 2018-2020 Buick Envision, were found to lack the required low-tensile thread. The applicable part numbers were immediately placed in quarentine status and the shipment volumes were added to this recall notification. The seat cover in question is model specific. A design review utilizing the automotive safety device database subscription (“DataOne”) determined that no other design configurations were identified lacking the required low-tensile thread when airbags are present in the seat. Description of Remedy : Description of Remedy Program : The manufacturer will be sending out newly manufactured seat covers with the required low-tensile thread. How Remedy Component Differs from Recalled Component : The manufacturer approved replacement seat cover will include lowtensile thread sewn in the outboard side of the seat cover. Instructions will also be provided to facilitate removal of the recalled seat covers and installation of the remedied seat covers. Identify How/When Recall Condition was Corrected in Production : Existing inventory will be destroyed. All new production of the affected models, after August 24, 2023, will include the required low-tensile thread on the outboard side of the seat cover. Recall Schedule : Description of Recall Schedule : The company will send the attached draft communication by email (when available) and USPS to all customers of record after NHTSA review and, ideally, no later than seven (7) business days from this submission. Because most purchases are individual purchases utilizing the Covercraft website, Covercraft does not foresee challenges in reaching the end users. Covercraft is initiating a voluntary recall of these seat covers to infonn you of the situation and to advise you of the product improvement being initiated and made available. Covercraft is providing, at no charge, a replacement set of seat covers for the second row to replace the set that you purchased. The product will look similar to your existing seat cover and will install in the same manner.
