Unexpected Turntable Rotation from Gearbox Failure
Gearbox failure may result in unexpected turntable rotation, increasing the risk of injury.

Summary
Altec Industries, Inc. (Altec) is recalling certain 2023 DC and DM Digger Derricks. The turntable rotation gearbox may fail at high loads due to a defect in the brake flange casing.
Remedy
Altec will work with the gearbox supplier to test and replace the gearbox, if necessary, free of charge. Owner notification letters are expected to be mailed December 15, 2023. Owners may contact Altec’s customer service at 1-877-462-5832. Altec’s number for this recall is MAB 3120.

Chronology :
A gearbox on an Altec digger derrick
failed during production unit testing on July 13, 2023. The gearbox was returned to Tulsa Winch (TWG) for evaluation.
A second gearbox failed during unit testing in the same manner on August 7, 2023. All gearboxes supplied by TWG were placed on hold to avoid installing on any more units.
TWG inspected all Altec inventory and work in process with an ultrasonic tester on August 14, 2023. No other gearboxes were identified by the ultrasonic testing to be outside of the acceptable range. TWG continued to work with their supplier to identify the lot size of the affected component.
A third gearbox failed during unit testing on August 30, 2023. This gearbox had been within the original acceptable ultrasonic range causing the team to evaluate the ultrasonic testing values. The range of acceptable measurements was reduced.
After reviewing Altec’s unit test procedures, TWG informed Altec that Altec’s unit testing would cause any nonconforming product to fail.
On October 2, 2023, after reviewing field data, the Altec quality team identified a field return for a failed brake flange casing. The part was returned to TWG and confirmed to have ultrasonic measurements outside of the acceptable range. This gearbox had been installed in May of 2023.
On October 10, 2023, a potential field action investigation was opened to identify the potential field population of gearboxes that could have casings outside the specifications. After reviewing the production records, on October 13, 2023, Altec determined that a defect could exist on customer units.
There were no other warranty claims or field reports identified. There have been no accidents, injuries, or deaths related to this issue.

Part 573 Safety Recall Report 23V-702 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 Description of Remedy : Description of Remedy Program : For the gearboxes within the affected serial number range, Tulsa Winch (TWG) will do an ultrasonic test on each gearbox to identify components whose ultrasonic measurements are outside of the acceptable range. Any gearbox outside of the acceptable range will be replaced. The recall remedy will be completed free of charge when presented for repair. The Altec Warranty Policy covers the recall remedy, including cases requiring reimbursement for the cost of obtaining a pre-notification remedy of the problem associated with this recall. How Remedy Component Differs from Recalled Component : The replacement gearboxes have casting material that meets the specifications of the design. Identify How/When Recall Condition was Corrected in Production : On August 30, 2023, Altec contained all units and issued a corrective action to Tulsa Winch (TWG). TWG completed ultrasonic testing on all gearboxes in containment. TWG added quality checks to prevent any gearboxes from being built with a defective brake flange casing. In addition to the check put in place at TWG, the brake flange casing manufacturer has added quality assurance checks to their process.
