Limit Switch Improperly Installed

Summary
Terex South Dakota, Inc. (Terex) is recalling certain 2014-2019 TM bucket/boom trucks. The limit switch was installed upside down and does not stop platform level movement if the platform level exceeds 15 degrees.
Remedy
Terex will notify owners, and dealers will properly install the limit switch, free of charge. The recall began April 14, 2020. Owners may contact Terex customer service at 1-605-882-4000. Terex’s number for this recall is SN681.

SUBJECT: Leveling Sensor Installation
Issue:
On Terex TM aerial devices, manufactured from February 2011 through April 2020, the basket level limit sensors at the boom tip may have been assembled in the incorrect orientation. The limit sensors prevent the platform from going more than 15 degrees from level if the electrical system malfunctions or there is damage to components. The incorrect mounting of the level sensors may cause an out of level basket to continue moving more than 15 degrees out of level. If the leveling system malfunctions it may cause the occupants to fall from the platform.
Action:
What the Owner Must Do:
The owner must inspect their equipment and correct if required within 10 days of receiving this bulletin.
- Inform all users, operators, and supervisors that the basket level sensors must be inspected within 10 days on TM aerial devices and follow the continued use section of this bulletin until the unit is corrected. This bulletin applies to TM Aerial devices with the hydraulic generator at the boom tip and applies to machines with an end mount platform or dual side mount platforms.
- Inspect the unit to determine if your unit has the incorrectly installed level sensors.

- If your unit has the light colored potting visible on the top sensor, or the sensor labeled A on the bottom; this bulletin applies. Correct following the instructions in Field Service Kit Z1562. Take a picture before correcting the issue and another picture after correcting the issue. Send both pictures with the completion form including the serial number and date of correction to [email protected].
- If your unit has the light colored potting visible only on the bottom sensor with the wires coming out of the sensors above the mounting screw and with sensor labeled A on top this bulletin does not apply and use can continue without restriction.
Continued Use:
- Inform the users of the content of this bulletin and remind them of the daily pre-shift inspection requirements.
- If the leveling system malfunctions and the platform gets more than 15 degrees out of level utilize one of the following procedures to stop the leveling system:
- Release the enable lever and use the manual level adjustment control to correct the level of the platform.
- From the lower controls shut off the upper controls and use the lower controls to level platform and lower boom.
- Repair issue that caused the malfunction and correct leveling sensor per the Field Service Kit Z1562 instructions.
- Always follow inspection and maintenance requirements as specified in the manuals.
What Terex will Do:
Terex will provide a Field Service Kit, Z1562, to the nearest Terex Utilities dealer or Field Service Technician for installation on your unit at no cost to the owner.

Dealers and Installers: A letter is being sent to owners of affected units. If affected units are in your area TEREX Utilities will contact you to arrange the repairs. Affected units, as shown on the owner letter, will have the repairs performed at no charge to the owner. Labor allowance is $120.
Only TM models built with hydraulic generators at the boom tip are affected. If the owner contacts you call TEREX Utilities Warranty Department at 1-844-837-3948 for repair instructions.
Important: Some of the involved units may be in rental fleets. Federal Law requires you to complete the recall service on these units before renting, or to inform the renters within 10 days of receipt of this bulletin.

SN681 is for the platform leveling system on Terex TM end mount aerial devices. One of the two basket level limit sensors at the boom tip may have been assembled upside down. The limit sensors prevent the platform from going more than 15 degrees from level if the electrical system malfunctions. The incorrect mounting of the level sensor may cause an out of level basket to continue moving more than 15 degrees out of level. If the leveling system malfunctions it may cause the occupants to fall from the platform, increasing the risk of an injury.
WHAT TEREX WILL DO
No parts are required to reposition the sensor if required. Terex will provide Field Service Kit Zl 562, sensor repositioning and testing instructions, and labor at no charge to the customer. with the positioning of the platform leveling sensors at the boom tip. NHTSA has assigned their number 20V103 to this
bulletin.
Please dispose of any of the previous Safety Notice 681 information you received. Dispose of the previous safety notice
bulletin, work instructions, and SN681 work completion form.
I’ve attached the SN681A bulletin, Z1562 FSK bill of material plus work instructions, and the SN681A work completion
form. I’ve also attached the owner lists for U.S. units, Canadian units, and export units. Note also Column K. If a
customer ordered a Z1390 FSK/TM TILT CNTRL RETROFIT as a replacement part, they are affected as well. The Z1390 FSK
included the same level sensors so the machines that they were installed on are affected by this notice as well. The
ordering customer purchase order number is listed. The projected owner letter mailing date for U.S. owners is Tuesday
the 23d. The mailing date for Canadian customers is to take place no later than the 23d also.

5) SWITCH “A” IS WIRED TO A6 AND SWITCH “B” IS WIRED TO A7
6) CONNECT THE POSTIVE LEAD OF A VOLTAGE METER TO PIN A6 AND THE NEGATIVE LEAD TO PIN A4. DO NOT
DISCONNECT THE WIRES IN PIN A4 AND A6.
7) ENABLE THE SINGLE STICK AND MANUALLY LEVEL DOWN (DUMP) THE PLATFORM BY MOVING THE MANUAL LEVER
ON THE SERVO VALVE SIDE TO SIDE. DO NOT LIFT UP ON MANUAL LEVER ON SERVO VALVE OR THE LEVER COULD
BREAK.
8) VERIFY THERE IS 12 VOLTS ACROSS PIN A4 AND PIN A6 WHEN THE PLATFORM IS MORE THAN 10 DEGREES DOWN.
9) CONNECT THE POSTIVE LEAD OF A VOLTAGE METER TO PIN A7 AND THE NEGATIVE LEAD TO PIN A4. DO NOT
DISCONNECT THE WIRES IN PIN A4 AND A7.
10) ENABLE THE SINGLE STICK AND MANUALLY LEVEL UP (TIP UP) THE PLATFORM BY MOVING THE MANUAL LEVER ON
THE SERVO VALVE SIDE TO SIDE.
11) VERIFY THERE IS 12 VOLTS ACROSS PIN A4 AND PIN A7 WHEN THE PLATFORM IS MORE THAN 10 DEGREES UP.
LEVEL CONTROLLER
12) ENABLE THE SINGLE STICK AND MANUALLY LEVEL THE PLATFORM WITH THE MANUAL LEVER ON THE SERVO VALVE
TO ZERO DEGREES. CONNECT B1 AND B2 (RE-ZERO LEVEL) TERMINALS TOGETHER WITH A FLAT BLADE SCREWDRIVER
FOR 5 SECONDS. LEVEL CONTROLLER LED WILL TURN RED AND THEN BACK TO FLASHING GREEN.
