Improperly Crimped Lifting Cable
An improperly crimped cable may come out of place during a lifting operation, possibly causing the generator set to fall, increasing the risk of injury.

Summary
Sutphen Corporation (Sutphen) is recalling one 2020 Fire Truck equipped with a Cummins
generator set. The lifting cables may be improperly crimped.
Remedy
Cummins will inspect the lifting cable, and either replace the cable or install a warning label, free of charge. Owner notification letters were mailed in October 29, 2021. Owners may contact Sutphen’s customer service at 1-614-889-1005.

Chronology :
May 13, 2021 โ An incident occurred at the Cummins assembly plant during which an alternator fell onto an assembly table after the lifting cable pulled out of the ferrule. No injury occurred. That same day, an OEM customer notified Cummins that a unit fell while being lifted at the OEM facility. Cummins initiated a stop ship/stop build and instructed account managers to notify affected OEMโs not to use the cable to lift the gensets pending further investigation of the issue.
May 13 โ June 21, 2021 – Cummins conducted an investigation into the issue, including working with the supplier to conduct tests on the cable crimp to evaluate the resulting lifting capability.
May 27, 2021 โ Cummins began production with properly crimped cables.
June 17 โ July 27 โ Cummins assessed the safety risk of this condition.
July 21, 2021 โ Cummins published a quality campaign to the field. (TSB210146-07132021 was submitted to NHTSA on August 6, 2021, in accordance with 49 CFR Part 579.5.)
August 10, 2021 โ Based upon the results of the investigation, Cumminsโ Product Safety Defect Board decided to conduct a safety campaign to address this condition in the subject population. To date, there have been no injuries related to this condition.
