Descriptive Information : The recall scope was determined by the 3rd party test reports that we have with
passing 3rd party testing reports that WS001 size large passed penetration tests.

Description of Defect :
Description of the Defect : Human error in the manufacturing of the shell.

Description of the Safety Risk : In the event of a crash resulting in direct impact to the helmet, this helmet
may not provide adequate protection to the wearer, resulting in injury.
Description of the Cause : We are currently processing this request.

that can Occur :
In the event of a crash, while unlikely, this helmet may not provide adequate
protection to the wearer from direct impact.

Provide the chronology of events leading up to the defect decision or test date for non-compliance decision:
• November 25, 2020 – NHTSA notified Fuel Helmets of failed impact test reports.
• December 2, 2020 – Sent email to have all SH-WS0016 helmets placed in “Quarantine” status – total
105 helmets. This process makes sure helemts cannot be sold.
• January 2021 WS001 Size large was tested using SGS 3rd part test – Results passed
• October 26, 2021 – meeting with NHTSA to discuss recall.
• Decemeber 9, 2021 – meeting with Fuel attorney group – Decision to move forward with voluntary
recall.
Description of Remedy :
Description of Remedy Program : Helmet Company will establish measures in place to insure customers who
have a helmet affected by this recall able to receive credit toward the
purchase of a different Fuel Helmet or reimbursement of the purchase
price . Customer service phone and email contact points will be put in
place to insure ease of contact and customer service.

After informing TAO TAO factory of the issue, Fuel Helmets, Inc., decided
to quarantine balance of 103 size large helmets in inventory and remove
all sales listing.
