Platform Leveling Cylinder Support Welds Failure

Summary
Terex South Dakota, Inc. (Terex) is recalling certain 2019-2022 TL80, TL80/112, and TL100 aerial devices. The welds for the platform leveling cylinder support bosses may crack, which can cause the cylinder support pin to fail and allow the platform to tip.
Remedy
Dealers will install reinforcement plates to the boom tip platform leveling cylinder bosses, and, all TL80/112 aerial devices will have the hydraulic storage latch for the elevator replaced, free of charge. Owner notification letters were mailed on October 13, 2022. Owners may contact Terex’s customer service at 1-605-882-4000. Terex’s number for this recall is SN712.

Issue:
Terex model TL80, TL80/112, and TL100 aerial devices, produced from March 2019 through August 2022, may have the welds for the boom tip leveling cylinder support boss fail. Once the welds fail, the cylinder base pin may flex repeatedly until it cracks, and it may fail. Failure of the cylinder base pin may result in the platform tipping, causing the platform occupants to fall resulting in injury.

The owner must inspect their equipment and correct if required.
- Immediately inform all users, operators, and supervisors of the content of this bulletin.
- Inspect for cracks of the boom tip leveling cylinder boss to boom welds, using Figure 1 as reference.
- If the welds are not cracked, see the Continued Use section in this bulletin.
- If the welds are cracked, do not use the machine until Field Service Kits Z1665 and Z1666 (TL80/112 only) are installed.

Inform the users of the content of this bulletin and remind them of the daily pre-shift inspection requirements.
- Visually inspect the boom tip leveling cylinder boss welds daily before use until Field Service Kits Z1665 and Z1666 (TL80/112 only) are installed.
- If the welds are not cracked, machine use can continue until Field Service Kits Z1665 and Z1666 (TL80/112 only) are installed.
- If the welds are cracked, do not use machine until Field Service Kits Z1665 and Z1666 (TL80/112 only) are installed.
- Contact Terex Utilities using the contact information in this bulletin to arrange repair using the following guidelines:
- For TL80/112 models, contact Terex Utilities within 10 days of receiving this bulletin to schedule the installation of Field Service Kits Z1665 and Z1666.
- For TL80 and TL100 models, contact Terex Utilities at the next scheduled maintenance interval for the installation of Field Service Kit Z1665.
- Always follow inspection and maintenance requirements as specified in the manuals.
What Terex will Do:
Terex will provide for all TL80, TL80/112, and TL100 machines Field Service Kit Z1665, parts and six (6) hours labor, at no cost to the customer. Field Service Kit Z1665 will add reinforcement plates to the boom tip leveling cylinder bosses inside the upper boom. For all TL80/112 machines, Terex will also provide Field Service Kit Z1666, parts and two (2) hours labor, at no cost to the customer. Field Service Kit Z1666 will add a ratcheting tie down for the hydraulic elevator at the base of the machine.

Field Service Kits Z1665 and Z1666 (TL80/112 only) will be provided to the nearest Terex Utilities dealer or Field Service Technician for installation on your unit.
Dealers and Installers: A letter is being sent to owners of affected units. If affected units are in your area TEREX Utilities will contact you to arrange the repairs. Labor allowance for Field Service Kit Z1665 is six (6) hours and for Field Service Kit Z1666 is two (2) hours.
Only TL80, TL80/112, and TL100 models are involved. Field Service Kit Z1665 will apply to the TL80, TL80/112, and TL100 models. Field Service Kit Z1666 only applies to the TL80/112 model. If the owner contacts you call TEREX Utilities Warranty Department at 1-844-837-3948 for instructions and to arrange shipment of parts.
Affected units, as shown on the owner letter, will have the repairs performed at no charge to the owner.
Important: Some of the involved units may be in rental fleets. Federal Law requires you to complete the recall service on these units before renting, or to inform the renters within 10 days of receipt of this bulletin.

Chronology :
In May of 2020, Terex was notified that welds for the boom tip platform leveling cylinder support bosses cracked. The cylinder support pin was not cracked. Due to the construction of the weld joint, the bosses with cracked welds were held captive by the boom structure, preventing the platform from tipping.
An investigation began. Terex released Product Notice PN689 on 5/6/21 (reported as an External Communication in May of 2021) informing owners to inspect the welds and add additional welds. Production booms had the additional welds added in June of 2020.
Between 4/8/22 and 4/26/22, Terex was notified of 3 machines that had the additional welds crack. The boom tips for 2 of the machines were sent back to Terex for analysis.
On 5/10/22 Terex was notified of a machine that during an inspection it was discovered that the additional welds and cylinder retainment pin cracked, which could result in the platform tipping. Development and validation of a fix began at that time.
On 8/30/22, the list of affected units was determined. The decision to perform a recall was made on 9/1/22. To date, there have been 8 machines reported with the additional welds that have cracked. Out of the 8 machines, 7 are the TL80/112 model and 1 is the TL100 model.
The analysis of the issue has determined that the welds are cracking during road travel and on the TL80/112 units due to the current hydraulic latch for the elevator not properly tying down the machine during travel.
The decision was made to recall the TL80/112 machines requiring the boom tip platform leveling cylinder support bosses be reinforced with additional plates and the elevator be secured with a ratcheting hold down as soon as possible.
The TL80 & TL100 models do not have the elevator and have seen a very low failure rate. Out of an abundance of caution it was decided to recall the TL80 & TL100 models and have the reinforcement of the boom tip leveling cylinder bosses added at the next maintenance interval.

The cylinder support pin was not cracked. Due to the construction of the weld joint, the bosses with cracked welds were held captive by the boom structure, preventing the platform from tipping. An investigation began. Terex released Product Notice PN689 on 5/6/21 (reported as an External Communication in May of 2021) informing owners to inspect the welds and add additional welds. Production booms had the additional welds added in June of 2020. Between 4/8/22 and 4/26/22, Terex was notified of 3 machines that had the additional welds crack. The boom tips for 2 of the machines were sent back to Terex for analysis. On 5/10/22 Terex was notified of a machine that during an inspection it was discovered that the additional welds and cylinder retainment pin cracked, which could result in the platform tipping. Development and validation of a fix began at that time. On 8/30/22, the list of affected units was determined. The decision to perform a recall was made on 9/1/22. To date, there have been 8 machines reported with the additional welds that have cracked. Out of the 8 machines, 7 are the TL80/112 model and 1 is the TL100 model. The analysis of the issue has determined that the welds are cracking during road travel and on the TL80/112 units due to the current hydraulic latch for the elevator not properly tying down the machine during travel. The decision was made to recall the TL80/112 machines requiring the boom tip platform leveling cylinder support bosses be reinforced with additional plates and the elevator be secured with a ratcheting hold down as soon as possible. The TL80 & TL100 models do not have the elevator and have seen a very low failure rate. Out of an abundance of caution it was decided to recall the TL80 & TL100 models and have the reinforcement of the boom tip leveling cylinder bosses added at the next maintenance interval. Part 573 Safety Recall Report 22V-674 Page 3 The information contained in this report was submitted pursuant to 49 CFR ยง573 Description of Remedy : Description of Remedy Program : All TL80, TL80/112, and TL100 machines will have reinforcement plates added to the boom tip platform leveling cylinder bosses. This kit will be added to the machine at no cost to the owner and will take 6 hours to complete. In addition, all TL80/112 machines will also have the hydraulic storage latch for the elevator replaced with a ratcheting hold down. This kit will be added to the machine at no cost to the owner and will take 2 hours to complete. How Remedy Component Differs from Recalled Component : The remedied machine will have the additional reinforcement plates added to the boom tip platform leveling cylinder bosses. For the TL80/112 machines, they will also have the hydraulic storage latch for the elevator replaced with the ratcheting hold down. Identify How/When Recall Condition was Corrected in Production : No affected machines have been built since the decision to recall on September 1, 2022. All upcoming machines will have reinforcement plates added for the boom tip platform leveling cylinder bosses and the TL80/112 machines will also have the ratcheting hold down for the elevator. Recall Schedule : Description of Recall Schedule : Dealer notification will begin by September 16, 2022.
