Liquid Transport Inc. located 14 axles from this order as uninstalled “loose” axles in our inventory. These 14 axles were remedied with parts supplied by Dexter Axle Company. The remaining 6 axles were located on three trailers (Model: PTNTT) (2 axles per trailer) manufactured and sold by Liquid Transport Inc. These were the only three trailers manufactured with axles containing potentially defective washers from the original order of 20 axles identified by Dexter Axle Company and Liquid Transport Inc. The trailers were identified and the owners were notified of the remedy proposed by Dexter Axle Company and supplied with replacement washers provided by Dexter. The potential leads to a hub seperation condition of the axle wheel end.” – Dexter Axle Company FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : “There is a potential for the washer to crack under load. If the washer cracks, the preload on the bearing is lost resulting in a premature bearing life and excessive end play in the hub assembly. Upon further investigation of the claims, it was discoverd the washer associated 42mm cartridge bearing washer was cracking leading to potential bearing and hub assembly performance issues including detachment from the axle.” – Dexter Axle Company Part 573 Safety Recall Report 22V-700 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 Description of Remedy : Description of Remedy Program : “Dexter instituted a 100% comformance testing of the hardness of the 42mm cartridge bearing washer on 18-Jun-2022. Dexter will contact each customer to coordinate repair/replacement of suspect axle hubs.” – (Dexter Axle Company)— Liquid Transport Inc: 20 Axles that were purchased from Dexter Axle Company contained the potentially defective component. Liquid Transport Inc. located 14 axles from this order as uninstalled “loose” axles in our inventory. These 14 axles were remedied with parts supplied by Dexter Axle Company. With this information provided to the Owners, Dexter also included recall re-imbursement instructions for Labor involved in replacing the correct replacement washers provided by Dexter Axle Company. How Remedy Component Differs from Recalled Component : “42mm cartridge bearing washers have been marked with a ‘gold’ paint dot to indicate the material hardness meets the product specifcations.” – Dexter Axle Company Identify How/When Recall Condition was Corrected in Production : “The corrective/containment actions were put into production on 20- Jun-2022.” – Dexter Axle Company Recall Schedule : Description of Recall Schedule : 20 Axles that were purchased from Dexter Axle Company contained the potentially defective component.
