) is recalling certain 2021-2023 Zero Radius trucks. The ejector welds may fail, causing pressure and force to be placed on the hydraulic cylinder, which can cause it to penetrate through the side of the truck body.
Remedy
McNeilus
will either send service representatives to repair affected vehicles or ship parts to customers, free of charge. Owner notification letters are expected to be mailed on November 21, 2022. Owners may contact McNeilus

Purpose:
McNeilus
identified the Zero Radius Side Loaders were manufactured with ejector welds that do not meet requirements. If the ejector sustains a failure, a cylinder may come through the side of the body, making the vehicle over width as well as potentially injuring anyone in the area around. These instructions describe the inspection process and schedule regarding the ejector welds.
Affected Vehicles:
The affected vehicle population is McNeilus
Zero Radius Side Loaders, produced between 10/01/2020 and 08/01/2022. Perform the procedure to only the vehicles on this VIN list.
Required Action:
Review and verify your ownership of all affected serial numbers of McNeilus
vehicles. If McNeilus
records indicate that you are the owner of one or more affected units, a Serial Number List is enclosed. Please be reminded that it is a violation of Federal law for you to sell or lease the vehicles covered by this notification until this recall has been performed on these vehicles. Substantial civil penalties apply to violations of this law.
Chronology :
In June 2022, McNeilus
received a report of a truck that had a hydraulic cylinder penetrate through the side of the truck body. McNeilus
reviewed the incident, and it was believed to be an isolate event as McNeilus
had not previously received any similar reports.
In July 2022, McNeilus
received a report relating to a different truck describing a similar situation and initiated a detailed investigation in conjunction with the parts supplier. The investigation remained ongoing through September 2022, during which McNeilus
conducted a thorough review and audit of the parts supplier’s manufacturing processes and facilities and failure modes. The investigation found that the ejector being produced by the supplier was not being produced to McNeilus
’ internal specifications including because the ejector was missing welds, the welds were of poor quality or incorrectly sized.
In August 2022, the supplier made an update to its processes to ensure McNeilus
’ specifications for the welds were being met in production. McNeilus
continued to review the issue including the factors affecting whether and how the ejector welds could separate and the potential consequences of an ejector weld failing, in particular the points on the truck body at which the hydraulic cylinder was located and could possibly penetrate the body.
On September 19, 2022, McNeilus
decided to conduct a recall to address affected vehicles in the field. McNeilus
is not aware of any injuries potentially related to this condition. While refuse truck operators do not conduct operations near this area of the truck, there remains some possibility that individuals could be located around the side of the truck and if they were to be present at the time of a failure, this could increase the risk of injury.

received a report of a truck that had a hydraulic cylinder penetrate through the side of the truck body. McNeilus
reviewed the incident, and it was believed to be an isolate event as McNeilus
had not previously received any similar reports. In July 2022, McNeilus
received a report relating to a different truck describing a similar situation and initiated a detailed investigation in conjunction with the parts supplier. The investigation remained ongoing through September 2022, during which McNeilus
conducted a thorough review and audit of the parts supplier’s manufacturing processes and facilities and failure modes. The investigation found that the ejector being produced by the supplier was not being produced to McNeilus
’ internal specifications including because the ejector was missing welds, the welds were of poor quality or incorrectly sized. In August 2022, the supplier made an update to its processes to ensure McNeilus
’ specifications for the welds were being met in production. McNeilus
continued to review the issue including the factors affecting whether and how the ejector welds could separate and the potential consequences of an ejector weld failing, in particular the points on the truck body at which the hydraulic cylinder was located and could possibly penetrate the body. On September 19, 2022, McNeilus
decided to conduct a recall to address affected vehicles in the field. McNeilus
is not aware of any injuries potentially related to this condition. While refuse truck operators do not conduct operations near this area of the truck, there remains some possibility that individuals could be located around the side of the truck and if they were to be present at the time of a failure, this could increase the risk of injury.
