Chronology :
On June 9 2022, Shyft received a complaint of the rear brakes overheating on the rear drive axle. On June 10, 2022, Shyft received a complaint involving the inadvertent application of the park brake. At that point, Shyft, in conjunction with the final stage manufacturer, conducted an inspection at their location of all coaches to determine if the valve was installed, and plumbed correctly. No non-conformances were found. However, Shyft continued to investigate by reviewing build records for the two complainant vehicles to understand if there was a change in personnel (reviewing operator sign-off). No change in personnel was identified. On September 7, 2022, an issue was identified in production that indicated the valve could be installed incorrectly and therefore subject to incorrect plumbing. Shyft continued to evaluate the issue and on October 13, 2022, in an overabundance of caution, Shyft determined a safety related defect could exist in the affected vehicles.
