The following procedure outlines how to remedy this situation by replacing the steering arm with an up-dated version from the manufacture. Clean mounting surface and surrounding surfaces to prepare for mounting of the new bolt-on steer arm and additional welds. 3. Grind a 3/16” chamfer along the mounting face edge. 4. Brenner Tank has decided that a defect, which relates to motor vehicle safety, exists in certain 2020-2021 model year Brenner Tank vehicles. Description of Defect The steer aim on the IMT lift axle has potential to fail. Risk to Motor Vehicle Safety: The fractured steer aim and tie rod can separate and fall to the ground, increasing the risk of crash or injmy. The equipment listed within should be inspected and remedied per the instructions described in TSBIMT1001 as soon as practical. Letter gave our PO# of three axles affected in the recall for Brenner trailers. Started the investigation to determine our part number. Scheduled emergency meeting with our Product Safety Team. 10/28- Held meeting with our Product Safety Team. Vote was to escalate and meet with Product Safety Committee. Scheduled emergency Product Safety Committe meeting. 10/31- Determined the three trailers, vin numbers and customer. I did reach out to Phil Schnurr who is the contact for Dexter in regard to the number of units affected. Our system shows we purchased five axles. Three were built at our Brenner facility and two were built at our Bulk facility. In talking with Phil they did not send a letter to our Bulk facility as they didn’t think they needed to. All five trailers sold to the same customer. Phil said he would support repairs on all five units. 11/1- Held Product Safety Committee meeting. Vote was to proceed with recall process for all five trailers. I started the 573 documents. 11/2- Completed 573 and submitted. 11/4-Recevied e-mail from Sarah Shiver with necessary changes for the Brenner trailers. I made the same changes to these 573 documents. I resubmitted the 573. The letter gave our PO# of the axles affected in the recall. Started an investigation to determine our part number and identify what trailers have these steer arms installed that were manufactured at the Brenner facility in Wisconsin. 10/28- Held meeting with the Product Safety Team, which voted to escalate the matter to the Product Safety Committee. Scheduled Product Safety Committee meeting. 10/31- Discovered that steer arms with the same manufacturing flaw were shipped to the Bulk Tank International manufacturing facility in Mexico, but were not listed in the letter from Dexter. Contacted Dexter and learned they did not identify the steer arms shipped to Bulk because they were shipped to Mexico and thus Dexter did not think they needed to include these components in their equipment recall population. 11/1- Held Product Safety Committee meeting. Committee voted to proceed with a safety recall to remedy 5 defective steer arms, 3 installed in the Brenner Tank facility in Wisconsin and 2 installed in the Bulk Tank facility in Mexico and sold in the United States. All 5 trailers were sold to one customer. 11/2- Submitted a 573 safety recall report to NHTSA for the two trailers manufactured by Bulk Tank International. (See 22V-821 for the 573 report for the 3 trailers manufactured by Brenner.) 11/4-Revised 573 per input from NHTSA RMD 3/29/2023 – It was brought to our attention that repair parts were not available from the supplier, Dexter Axle, at the time the owner was notified of this safety recall in January 2023. Accordingly, the first notification letter should have been an interim letter. Parts are now available, and a final owner notification letter will be issued by 05/01/2023 Part 573 Safety Recall Report 22V-822 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 Description of Remedy : Description of Remedy Program : New steering arms will be installed under warranty and at no cost to the customer.
