Production Dates : SEP 12, 2022 – NOV 04, 2022 VIN Range 1 : Begin : NR End : NR Not sequential Description of Defect : Description of the Defect : During turn signal activation, a side view image of the vehicle is displayed to the driver. When a side view image is being displayed and the brake is applied, the image could reverse showing the opposite side of the vehicle. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : A reversed image could cause the driver to lose site of pedestrians or other vehicles on the side of the vehicle being shown. This could could increase the risk of injury or crash. Description of the Cause : Monitor harness was modified and installed incorrectly during manufacturing. Identification of Any Warning that can Occur : The side view image can reverse without warning. Involved Components : Part 573 Safety Recall Report 22V-846 Page 2 The information contained in this report was submitted pursuant to 49 CFR §573 Component Name 1 : Monitor Component Description : MONITOR 7″ RVS PD-062710 Component Part Number : 16518539-062710 Supplier Identification : Component Manufacturer Name : NR Address : NR NR Country : NR Chronology : On, or near 11/4/2022 Shyft/UTM became aware of a condition in which the side view image may be reversed in certain vehicles. Teams began investigating and learned the condition would occur when the service brake is applied. A ship hold was implemented on the same day. Shyft/UTM continued its investigation and found a the monitors had not been installed per design intent. On, 11/11/2022 the number of affected units were identified and Shyft determined a safety defect could exist in the affected vehicles. Description of Remedy : Description of Remedy Program : A jumper harness will be installed to bring the installation back to designed intent. This will be done at no charge. Vehicles affected by this recall are still within their warranty period and any repairs would be covered under the normal warranty. How Remedy Component Differs from Recalled Component : Vehicles subject to the remedy will have the jumper harness installed between the monitor and main electrical harness, or the monitor will be connected directly to the main harness as designed. Identify How/When Recall Condition was Corrected in Production : A ship hold was issued on 11/4/2022 and those vehicles off line had a jumper harness installed. Those vehicles still on the assembly line had the monitor connected to the main harness per design.
