This will affect the lock valve cartridge seals and may allow the cylinder to drift. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : Drifting of the cylinder may result in the machine tipping resulting in injury. Description of the Cause : The cylinder may have been machined incorrectly, affecting the lock valve cartridge seals and allowing the cylinder to drift. Identification of Any Warning that can Occur : The operator is to conduct a daily pre-shift operational check of all outrigger and boom functions. Drifting of an outrigger could be detected during this check. The investigation and consultation with the vendor determined that the cylinders may not have been machined correctly. The decision to recall the cylinders was made on November 22nd. Description of Remedy : Description of Remedy Program : All affected 631148 cylinders will be replaced with new, correctly machined cylinders. The cylinders will be replaced at no cost to the owner. How Remedy Component Differs from Recalled Component : The remedied cylinders start with cylinder serial number XH22110001 and progress numerically (XH22110002, XH22110003, etc). The recalled cylinders are serial numbers XH22090001 through XH22090234 and XH22100001 through XH22100100. Identify How/When Recall Condition was Corrected in Production : The potentially defective cylinders were isolated in production on October 28, 2022. Dealers were contacted and instructed to isolate all possibly defective cylinders. Twelve potentially defective cylinders were not found. There were 17 completed units containing 40 cylinders shipped by the installers to the end customers, which will contain the 12 remaining potentially defective cylinders. We combined the units in question and added the installation tie down drawing for reference. These tie down drawings are also attached. Per our discussion, the cylinders are ident ifiable by: • They are painted black from the supplier (The exposed portion has likely been painted over but the original paint may be visible further down inside the outrigger leg} • A serial number starting with the prefix XH is stamped near the two fittings where the hoses hook up. • A cast seam is visible running between the two fittings (see the attached picture} Feel free to call with questions. On October 27th, shipments of outriggers containing cylinder part number 631148 were halted and cylinders on-hand were quarantined. The investigation and consultation with the vendor determined that the cylinders may not have been machined correctly. As part of the investigation, dealers were called and asked to inspect their stock and determine if there were any issues. The first shipment of corrected cylinders was received on November 14th. One dealer reported that they had sold units which could contain the affected cylinders. The dealer contacted the customers, inspected, and replaced all affected cylinders. The decision to recall the cylinders was made on November 22nd. On November 29th, an email was sent to a dealer with inspection criteria. By December 22, 2022 all potentially defective cylinders had been verified as being replaced with no reported operational issues. Email from NHTSA staff on February 6, 2023 stated that since all potentially defective cylinders were accounted for, an owner notification letter was not required, only a quarterly report was required showing the campaign was 100% complete.
