A Quality stop was issued on that date to stop shipment from Terex until all potentially affected booms could be inspected. By November 22, 2022 all potentially affected booms at Terex SD facilities were inspected and affected booms identified for correction. On November 22, 2022, a crack was identified on a completed machine at a customer site. The decision to recall was made on November 23, 2022. Description of Remedy : Description of Remedy Program : All potentially affected booms are to be inspected for cracking. Booms determined to be affected will be repaired or replaced at no cost to the owner. How Remedy Component Differs from Recalled Component : No visible distinguishing characteristics Identify How/When Recall Condition was Corrected in Production : On November 29, 2022, the weld machine was taken out of service and replaced with a new machine. Testing of welds with the replacement welder showed the issue was corrected. Production resumed on November 29, 2022 after the successful testing of welds performed with the replacement machine. This letter is to explain why the required owner’s letter mailing date was missed and what steps have been taken to prevent a reoccurrence in the future. The required mailing date was missed due to additional data being gathered to determine if there was a need to expand the recall population. When it was decided by Terex South Dakota, Inc. that the recall population would not change, all required documents were submitted to NHTSA for review and approval within one day of this decision. After approval of the documents was obtained, the mailing was completed within two days of the approval. To prevent missing the 60 day notification requirement in the future, Terex South Dakota, Inc. has developed a recall tracking tool to calculate the required mailing date based on the Defect Information Report submittal date. This tool will also flag when the mailing is within seven days of the required date as a visual reminder that the due date is approaching.
