Crosby supplied part numbers and shipment dates that were traced to the affected vehicles with the hoist rings installed. Quantity 2 – Gladiator 2018 trucks affected. Quantity 1 – Gladiator 2019 truck affected. Quantity 1 – MetroStar 2020 truck affected. Quantity 1 – Gladiator 2021 truck affected. Production Dates : NOV 30, 2017 – OCT 21, 2021 VIN Range 1 : Begin : NR End : NR Not sequential Description of Defect : Description of the Defect : Based upon information provided by Crosby, a few instances of the rope rescue anchor point (hoist ring), from the identified part numbers, have an important assembly component missing. In those instances, the retaining pins used to secure the hinge pins were not in place and allowed a hinge pin to come out during unloaded condition. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : If the hinge pin comes out of the hoist ring, it could result in potential injury. Description of the Cause : Manufacturing defect caused by the supplier of the hoist ring. Involved Components : Component Name 1 : HR-125/125M Frame 2 Hoist Ring Component Description : Hoist Ring – Rope Rescue Anchor Point Component Part Number : 1016644 Supplier Identification : Component Manufacturer Name : Crosby Address : NR NR Country : United States Chronology : On May 12, 2023 – Spartan Fire received communication from the hoist ring distributor for awareness of a Crosby inspection notice of a potential issue with their hoist ring assembly. May 16-17, 2023 – Spartan Fire Engineering was notified of the issue and an investigation was started. May 24-25, 2023 – Spartan Fire created a PSR Team, identified a potential population, and completed a risk assessment. On May 26, 2023 – Escalated the issue to REV Group Management and REV’s senior leadership approved a voluntary recall for this issue. Description of Remedy : Description of Remedy Program : Spartan will work with affected vehicle owners to inspect and replace the hoist rings. Spartan will reimburse the customer, if the component was replaced prior to notification. How Remedy Component Differs from Recalled Component : The remedy component would be the same. Spartan reviewed and did not find any product in inventory in May 2023.
