We were notified by our customer on 11/4/2022 that a manufacturing defect was discovered on our product during the installation process at their facility. At that time, additional safeguards and inspections were put into place at our facility to ensure no further defective product was shipped. To date, we have shipped 9,734 seats. 8,853 were shipped prior to implementing the additional safeguards and inspections. Production Dates : FEB 12, 2021 – NOV 04, 2022 Description of Defect : Description of the Defect : The passenger side flip-up seat may be missing a retaining ring that prevents the pivot rod from sliding. Absence of the ring could allow the rod to shift which could result in sudden loss of seat cushion support for the occupant. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : Sudden loss of seat cushion support could increase the risk of an injury. Description of the Cause : NR Identification of Any Warning that can Occur : Loss of support could occur without warning. Part 573 Safety Recall Report 23E-053 Page 2 The information contained in this report was submitted pursuant to 49 CFR §573 Involved Components : Component Name : R-Ring Component Description : 5/8″ Retaining Ring Component Part Number : 707616 Supplier Identification : Component Manufacturer Name : Seats Incorporated Address : 1515 Industrial Street Reedsburg Wisconsin 53581 Country : United States Chronology : On November 4, 2022, Seats, Inc. was notified by Shyft Group of a flip cushion that was found to be missing a retaining ring during installation and inspection of the seat function in one of their cabs. In February 2023, an additional customer, Capacity of Texas (Rev Group), was identified as having potentially been affected by this defect. Product was returned to Seats Incorporated for inspection and additional seats were found in that population that were missing one of the retaining rings. Description of Remedy : Description of Remedy Program : Seats Incorporated is working directly with both customers affected by this defect to identify any defective products and replace any missing or improperly installed retaining rings. How Remedy Component Differs from Recalled Component : The remedied product has both retaining rings properly installed on the pivot rod. Identify How/When Recall Condition was Corrected in Production : 100% inspection and validation of proper function was implemented on November 4th, 2022 for any new production components produced at Seats Incorporated. For product produced prior to that date, Seats Incorporated is currently working with our customers to identify and correct any defective product that has been sold.
