Summary
Terberg Benschop B.V. is recalling one 2022 YT193 terminal tractor. The eight non-self-locking flange nuts that attach the steering front axle to the front axle suspension may come loose.
Remedy
A dealer will replace the non-self-locking nuts and add thread lock as necessary, free of charge. The owner notification letter is expected to be mailed in November 2023. The owner may contact Terberg customer service at 1-348-459-294. Terberg’s number for this recall is CAM0152.

Introduction
From the end of January 2023 until October 2023 flange nuts ( non self locking) were used to mount the front axle to the YTxx3 and DTxx3. This flange nut t26031374 can be recognized by a spanner size 32 mm.
For some unknown reason we have seen a few cases were the nuts did unwind themselves from the bolts.
Therefore we changed our production method for the YTxx3 as follows:
Use 4 times a non self locking nut with Loctite 243 on the rear side of the axle where space is limited, and 4 self locking nuts for the remaining positions.
For information: The non self locking nut has spanner size 32 and the self locking nut t26041075 has spanner size 36.
We changed our production method for the DTxx3 as follows:
Use 8 times self locking nut t26041075 with spanner size 36.
We reworked as much vehicles as possible that we had not shipped. For all vehicle with the 8 non self locking bolts that were delivered before we discovered this issue or before we could perform our rework action please see the recommended action below.
Documentation group of the steering system 90-02-01-444 RHD or 90-02-01-536 LHD was not updated with the correct nuts. The correct nuts are displayed in the group of the front axle mounting 60-01-02-158. The fact that the wrong nut was displayed in group 90-02-01-444/536 might have been confusing to some.
As stipulated in the Terberg Maintenance Manual TD1215 the correct mounting and tightening of bolts on the front and rear axles should be checked on the first, periodic and annual maintenance intervals. According to TD1215 the first interval should be performed after the first 100 operating hours of the vehicle.
Recommended Action
On all vehicles built with 8 flange nuts on the front axle a modification must be made. Please read the instructions below carefully.
All vehicles that were built using the 8 non self locking nuts are part of this campaign. (both DTxx3 and YTxx3)
For the YTxx3: when 4 self locking nuts and 4 non self locking nuts are mounted the machines was reworked/produced according to the most recent standard and is considered okay.
For the DTxx3: when 8 self locking nuts are mounted the machine is assembled to the correct standards and is considered okay.
1. Park the vehicle in a safe place.
2. Place wheel chocks on the rear wheels.
3. Disconnect the main switch and make sure no one can start the vehicle unintended
4. Check if the machine has 8 non self locking flange nuts, spanner width 32mm.
5. Change the 4 nuts on the front side of the axle to normal self locking nuts t26041075, torque these to 850Nm.
6. For YTxx3 (for DTxx3 go to step 7): Undo the other 4 flange nuts on the rear side of the axle, apply Loctite 243 and torque to 978Nm.
7. For DTxx3 (For YTxx3 go to step 8): Replace the 4 nuts on the rear side of the axle for self locking nuts t26041075, torque these to 850Nm.
8. Retorque nuts on the front side to 850Nm.
9. Mark all nuts with a marker after tightening.
Chronology :
January 28th 2023: Start use of non self locking nut on 8 positions on front axle in production due to health and safety issue with mounting the existing nuts.
October 9th 2023: we received report from our distributor in UK that on a number of non-highway vehicles the nuts were found loose.
October 10th 2023: a meeting was held to start internal investigation.
October 17th 2023: decision was taken to go back to self locking nuts, and on locations where self locking nut is not possible a thread locker (Loctite 243) must be applied. All used torques and documentation have been reviewed. Also the containment action to re-work all machines that are produced but not yet delivered is started.
November 6th 2023: it is clear which machines are modified before delivery in the containment action, and which vehicles were not. Start with preparing the Modification Bulletin.

Descriptive Information : This recall applies to all vehicles produced from January 28th till 2023 until
corrective action was taken in production in October 2023. All vehicles that have been
re-worked prior to shipment are excluded from this campaign.
In this period we have produced 723 vehicles, of which only one is a DOT registered
vehicle that is used on-highway in the US. Al other units are used off highway. These
are located as follows: 181 in the US and the remaining 541 outside the US.

Description of Defect :
Description of the Defect : From the end of January 2023 until September 2023 8 non self locking flange

Description of Remedy :
Description of Remedy Program : Customers will be actively be informed by their dealer, and the dealer will
come to the customer site to repair the vehicle free of charge.
Replace the non self locking nuts for self locking nuts on the positions
where there is space for the tooling
Add thread lock Loctite 243 to the nuts where there is no space for the
tooling to install the larger self locking nut.
Torque all nuts to the correct setting.
How Remedy Component Differs
from Recalled Component :
The non self locking nut will be replaced for a self locking nut. OR thread
lock Loctite 243 will be added.
Identify How/When Recall Condition
was Corrected in Production :
From October 17th the new working method: (4 self locking nuts and 4
other nuts with thread lock) was applied in production
