The unsecured rear axle may shift sideways, resulting in loss of control of the vehicle, thereby increasing the risk of crash.

Summary

E-One Incorporated (E-One) is recalling certain 2018-2020 Cyclone 2 and Typhoon ambulances. The lower shock mount bracket may cause the U-bolts to bind and break, possibly preventing the rear axle from remaining secured.

 

Remedy

E-One has notified owners, and dealers replace any of the affected lower shock mount brackets, free of charge. The recall began September 21, 2020. Owners may contact E-One customer service at 1-888-237-1751.

Rear Shock Bracket Inspection Procedure

1. Measure and record the U-bolt hole spacing for both the 1086779 driver side shock mount bracket and the 1086780 officer side shock mount bracket. See Fig. 1 for drawing showing the features to measure.

2. If the U-bolt hole spacing in both brackets measures correctly at 3.8+/-.1 inches, please return this sheet to E-ONE with the fields above filled out. No further action is needed.

3. If the U-bolt hole spacing in one or both brackets measures to be 3.4+/-.1 inches or if the U bolts are not parallel you have a part that requires replacement as well as the UBolts, please return this sheet to E-ONE to supply replacement brackets, U bolts and Nuts. The suspension print will include reinstallation instructions and torque values.

This notice is sent to you in accordance with the requirements of the National Traffic and Motor Vehicle Safety Act.

E-ONE has decided that a defect which relates to motor vehicle safety exists in certain models of the following emergency vehicles manufactured September 21, 2018 to April 30, 2020, equipped with Rear Shock Mount Bracket 1086779 and 1086780:

E-ONE – 2018 TYPHOON, 2019 CYCLONE, 2019 TYPHOON, 2020 TYPHOON

 

WHY IS A RECALL BEING CONDUCTED?

The holes in the lower shock mount bracket cause U-bolt binding when the U-bolts are passed through the lower axle seat which have different spacing. U-bolts will not achieve complete clamp force thereby reducing the effectiveness of the axle to suspension interface. Lack of proper clamp force may result in the shearing of the axle locating dowels allowing the axle to shift sideways resulting in loss of control of the vehicle, increasing the risk of a crash. Axles with incomplete Ubolt torque can making popping or creaking noise as the axle to suspension interface is moving. Design error was the cause.

The affected item is the Rear Shock Mount Bracket 1086779 and 1086780.

 

WHAT ARE WE DOING ABOUT THE PROBLEM?

Vehicles subject to this recall are to be inspected and repaired by an E-ONE certified dealer or technician. Trucks will be inspected for the presence of revision A bracket per instructions. If revision A brackets are present, E-ONE will provide new brackets and U-bolts. E-ONE will compensate the dealer or owner for installing new shock mount brackets and U-bolts, provided free of charge, if it has not already been replaced during normal maintenance. Installation of the new components will take approximately 2 man-hours.

TO: Affected E-ONE Dealers

E-ONE has decided that a safety defect exists in certain E-ONE CYCLONE and TYPHOON vehicles manufactured from September 21, 2018 to April 30, 2020 equipped with Rear Shock Mount Bracket 1086779 and 1086780. Accordingly, a recall to address this issue is being initiated effective September 10, 2020

The holes in the lower shock mount bracket cause U-bolt binding when the U-bolts are passed through the lower axle seat which have different spacing. U-bolts will not achieve complete clamp force thereby reducing the effectiveness of the axle to suspension interface. Lack of proper clamp force may result in the shearing of the axle locating dowels allowing the axle to shift sideways resulting in loss of control of the vehicle. Axles with incomplete U-bolt torque can making popping or creaking noise as the axle to suspension interface is moving.

Owners will be notified by mail about the recall and will be instructed on how to resolve the issue. Trucks will be inspected for the presence of revision A bracket per instructions. If revision A brackets are present, E-ONE will provide new brackets and U-bolts. E-ONE will compensate the dealer or owner for installing the new components provided free of charge. Installation of the components should take approximately 2 man-hours. There is no cost to owners for this recall.

E-ONE is providing, free of charge, the necessary components and instructions to rectify the issue and will pay labor costs for inspecting and replacing the included components that have not already been replaced during normal maintenance.

Please be reminded that it is a violation of Federal law for you to sell, lease or deliver a new or used CYCLONE and TYPHOON equipped with Rear Shock Mount Bracket 1086779 and 1086780 covered by this notification until this recall has been performed on these CYCLONE and TYPHOON trucks. Substantial civil penalties apply to violations of this law.
