Summary

Charles Machine Works, Inc. (Charles Machine) is recalling certain 2008-2020 Ditch Witch S4S, 2015-2021 Ditch Witch 025-1020, 2015 Ditch Witch 745-7900, 2009-2019 Ditch Witch ZS6S, and 2010 Ditch Witch ZS6S-1 trailers designed for carrying the FX20 vacuum excavator. The trailers may be missing side reflex reflectors near the front of the trailer. As such, these trailers fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, “Lamps, Reflective Devices, and Associated Equipment.”

 

Remedy

Charles Machine will notify owners, and dealers will inspect for side reflex reflectors, and add them as necessary, free of charge. The began on March 31, 2021. Owners may contact Charles Machine customer service at 1-800-654-6481.

Instructions

  1. Clean areas shown with alcohol.

 

  1. Apply yellow reflectors where indicated in first two photos as close to tongue as possible on the side of trailer frame. Do not apply on tongue. Reflector will be behind the existing decals (shown in second photo) on one side.

 

  1. Apply red reflectors where indicated in third photo on both sides of trailer. Do not apply reflector over weld on rear-facing locations.

Dear Ditch Witch Customer:

 

Purpose of This Notice

This notice is sent to you in accordance with the requirements of the National Traffic and Motor Vehicle Safety Act.  This letter is to notify you of a potential hazard with your trailer.   We have decided that a defect which relates to motor vehicle safety  exists in certain 2008-2021 Ditch Witch® S4S model trailers.  Your trailer may have been manufactured without all the  reflectors required by the applicable Federal Motor Vehicle Safety Standards.  If the reflectors are missing, there is an increased risk of a crash,  someone may be injured, and/or property may be damaged.

 

What You Should Do

Contact your local dealership «Dlr_Nm_1», phone number «Dlr_Phone» as soon as possible to have this corrected at no charge.  Do not use your trailer until this repair has been made.  The labor time necessary to perform this service correction is up to 30 minutes.  If you have sold your trailer or vacuum excavator, please advise your Ditch Witch dealership of the new owner.

 

Additional Information

Your Ditch Witch dealership is best equipped to obtain parts and provide service to ensure that your vacuum  excavator/trailer is corrected as promptly as possible. If, however, you take your vehicle to your dealer on the agreed service date, and they do not remedy this condition on that date or within three (3) days, we recommend you contact  Product Support at The Charles Machine Works, Inc., phone number 1-800-433-6652.

 

After contacting your dealership and The Charles Machine Works, Inc., if you are still not able to have the safety defect remedied without charge and within a reasonable time, you may wish to write the Administrator, National Highway Traffic  Safety Administration, 1200 New Jersey A venue SE, Washington, DC 20590 or call 1-888-327-4236 (TTY: 1-800-424-9153); or go to https://www.safercar.gov.

 

Federal regulations require that any vehicle lessor receiving this recall notice must forward a copy of this notice to the lessee within ten days.

 

If ownership of the listed equipment has changed, please help us update our records and notify new owners by completing the following information and returning this portion to us.
