Summary
Altec Industries, Inc. (Altec) is recalling certain 2021 AH125 aerial devices. When operating the boom positioning control lever, a defective linear potentiometer may cause the boom to move in an unintended direction.
Remedy
Altec will inspect the linear potentiometer manufacturer date, and replace the potentiometer if necessary, free of charge. Owner notification letters were mailed on March 16, 2022. Owners may contact Altec customer service at 1-877-462-5832. Altec’s number for this recall is CSN 847.

Dear Altec Owner,
This notice is sent to you in accordance with the National Traffic and Motor Vehicle Safety Act and Canada Motor Vehicle Safety Act.
Altec Industries, Inc. has decided that a defect which relates to motor vehicle safety exists in certain AH125 aerial devices built between March 2021 and November 2021. This is to inform you that your vehicle may contain a defect that could affect the safety of a person. If a unit has a linear potentiometer from the affected Lot, the failure could result in uncontrolled movement when operating from the upper controls. Death or serious injury could result from uncontrolled movement.
Refer to the included notice for the items covered under the Altec Warranty Policy. If you had this repair performed before you received this notice, you may be eligible to receive reimbursement for the cost of obtaining a pre-notification remedy of the problem associated with this recall. All work will be performed at no charge to the customer when presented for repair.
Compare your unitโs identifying information with the provided list to verify your unit is affected. You may also contact Altec or view your fleet through Altec Connect to determine if there are any other outstanding notices.
The inspection and repair can be performed by Altec. This work is estimated to take up to seven hours and one person to complete.
If you have sold or retired the unit, update the records through Altec Connect. If you have leased this equipment to another person or company, you are required by Federal Law to forward a copy of this notice to the lessee by first class mail within ten (10) days of the receipt of this notice.

Chronology :
On or around November 29, 2021, a routine review of warranty information identified a Service Request opened on October 3, 2021, to repair a customerโs unit with rotation controls not functioning correctly. A linear potentiometer was identified as the cause of the improper operation. The unit was repaired and returned to service. Two other Service Requests that were opened on November 8, 2021, required the linear potentiometer be replaced to repair an improperly functioning control handle.
On December 9, 2021, after the three SRs identified a defective potentiometer as a cause for unintended boom movement, Altec began investigating all service and warranty records relating to the involved linear potentiometer. This results of the investigation was provided to the supplier. The supplierโs investigation found that a specific Lot of potentiometers could potentially be defective. On January 28, 2022, after reviewing the Altecโs and the supplierโs investigative results, Altec determined that a defect could exist on AH125 units. There have been no accidents, injuries or deaths related to this issue.
