MV and PV vehicles. The walk board rack mounting bolts may loosen, allowing the rack to separate from the vehicle.
Remedy
KTM
will install new washers, bolts, and locking nuts, free of charge. Owner notification letters were mailed August 25, 2022. Owners may contact KTM

Chronology :
On April, 13, 2022 Kentucky Trailer
(KT) personnel were notified by a customer that a walk board rack mounted to its truck body was missing 4 nuts/bolts which caused it to became loose and led to the walk board itself falling from the truck body unit. The incident did not result in any personal injury or property damage, other than damage to the walk board itself. This was the first report of such an incident since KT began manufacturing truck bodies in 2014. It was initially suspected that the nuts/bolts had been intentionally removed in an attempt by someone to steal the walk board. However, KT then developed a plan to locate and test additional nuts/bolts on similar units that were operating in the field. KT sales/marketing team members reached out to certain customers in an effort to locate similar units that could be made available for inspection.
Given it is “moving season”, units that could be made available for inspection proved difficult to locate; however, on June 2, 2022 KT engineering and quality personnel inspected 2 truck body units manufactured in 2016 and 1 manufactured in 2017. The applicable nuts/bolts on 1 of the 2016 units and on the 2017 unit were all secure. However, certain nuts/bolts on the other 2016 unit were either loose or missing (KT replaced all nuts/bolts on this unit the same day). On June 3, 2022, KT engineering personnel inspected 4 KT truck body units (all manufactured in the 2020/2021 time frame) and confirmed all applicable nuts/bolts on the units were secure. On June 6, 2022, KT engineering personnel inspected 2 KT truck body units manufactured in 2018 and determined that certain nuts/bolts on the units were either loose or missing. On June 7, 2022, KT convened a meeting of certain management personnel and its board of directors and made a good faith determination that certain KT truck body units contain a safety related defect and that KT should proceed to notify NHTSA and initiate a recall.

Reimbursement Plan NHTSA Safety Recall No. 22V419 Pursuant to the requirements set forth in 49 C.F.R. Part 573 and Part 577 of the Code of Federal Regulations, Kentucky Trailer Manufacturing (“Kentucky Trailer”) is submitting information pertaining to our reimbursement plan, related to NHTSA Safety Recall No. 22V419 (the “Safety Recall”), for the cost of repairs paid for by vehicle owners to remedy the defect that is the subject of the Safety Recall (i) before they were notified of the Safety Recall, and (ii) subsequent to their being notified of the Safety Recall. Set forth below are the details of such reimbursement plan. Reimbursement Notification Kentucky Trailer will notify vehicle owners in accordance with 49 C.F.R. Part 577 that Kentucky Trailer is offering to reimburse vehicle owners for the cost of repairs paid for by vehicle owners to remedy the defect that is the subject of the Safety Recall (i) before they were notified of the Safety Recall, and (ii) subsequent to their being notified of the Safety Recall. An owner will be eligible for reimbursement for the cost of such repairs paid for by vehicle owners in each instance, pursuant to the terms of this reimbursement plan. Costs to be Reimbursed Costs to be reimbursement will be no less than: 1. In the case of repairs made before being notified of the Safety Recall, the amount paid by the owner for the remedy that specifically addressed, and was reasonably necessary to correct, the defect that is the subject of the Safety Recall; or 2. In the case of repairs made after being notified of the Safety Recall, the costs of the part(s) for the remedy (to be no more than the manufacturer’s cost to procure the authorized parts), plus associated reasonable labor charges (at local labor rates), miscellaneous fees (such as disposal of waste), and taxes. Entity Authorized to Provide Reimbursement Kentucky Trailer will reimburse owners through Kentucky Trailer’s Corporate Support Center and will notify owners that they may pursue requests for reimbursement directly through the Kentucky Trailer Support Center. Vehicle Owners will be directed to submit the below identified information and documentation, upon which reimbursement eligibility will be determined. Required Documentation Determination of eligibility for reimbursement of the cost of repairs paid for by vehicle owners to remedy the defect that is the subject of the Safety Recall will depend upon the information provided by the owner. Requests for reimbursement can be made by completing a Reimbursement Request Form found on Kentucky Trailer’s website (www.recall-kytrailer.com). This Reimbursement Request Form calls for the owner to provide the following information: 1. VIN # of Repaired Vehicle; 2. Name of Vehicle Owner; 3. Address of Vehicle Owner; and 4. Contact Information of Vehicle Owner. The completed Reimbursement Request Form must be submitted to Kentucky Trailer’s Corporate Support Center at: Kentucky Trailer, Attn: Recall – 2022KTB01, 3400 Robards Court, Louisville, KY 40218, and must be accompanied by an original receipt for the remedy that includes a breakdown of the amount paid for parts, labor, other costs and taxes. Where the receipt covers work other than to address the recall, Kentucky Trailer may require the claimant to separately identify costs that are eligible for reimbursement. Failure to submit all of the above information may result in denial of the reimbursement request. What will Kentucky Trailer do? Kentucky Trailer will remedy the subject defect by removing all applicable fasteners installed on the walk board rack assembly (~28) and replacing them with new fasteners (i.e. nuts/bolts/washers) that include Stover-type ” locking” nuts. Kentucky Trailer will undertake this repair for you (free of charge), effective immediately, at its Louisville, KY (3400 Robards Court, Louisville, KY 40218) or Fontana, CA ( 1014 7 Calabash Ave, Fontana, CA 92335) service facility. Alternatively, Kentucky Trailer will ship to you, or your designated service/repair facility (free of charge), a kit containing the fasteners required to complete the repair along with a copy of the attached detailed technical repair instructions. Service/repair facilities that customarily service vehicles of this type should be well equipped to complete this repair. ) filed a Defect and Noncompliance Report with the National Highway Traffic Safety Administration (NHTSA) informing the agency of our intent to conduct a voluntary Safety Recall. This Safety Recall covers all Kentucky Trailer
truck body units (models MV & PV) manufactured by Kentucky Trailer
between January 5, 2015 and May 31, 2022 that have a walk board rack mounted to the underbelly of the truck body unit. Description of Defect The walk board rack mounted to the underbelly of certain Kentucky Trailer
truck body units, which rack serves as the housing for each unit’s walk board while the unit is in transit, can become loose due to vibrations, damage and/or other forces loosening the 28 (nut/bolt) fasteners that serve to attach the walk board rack to the underbelly of the truck body unit. If enough fasteners become loose (or ultimately separated), there can be movement in the walk board rack sufficient to allow the walk board to fall off of the truck body unit. If a walk board were to fall off of a truck body unit, particularly if the truck body unit is in transit, serious injury and/or damage to nearby persons, vehicles and/or property (including the truck body itself) could occur. Remedy The subject defect may be remedied by removing all applicable fasteners installed on the walk board rack assembly (~28) and replacing them with new fasteners (i.e. nuts/bolts/washers) that include locking nuts (Stover brand). Kentucky Trailer
intends to offer this repair (free of charge) at its Louisville, KY and Fontana, CA service facilities. Kentucky Trailer
also intends to make available for shipment to owners and operators of Kentucky Trailer
truck body units (or their designees), packages containing the fasteners necessary to complete the repair along with detailed technical repair instructions. Kentucky Trailer
intends to reimburse owners and operators for the reasonable cost associated with making such repairs (which repairs are expected to take <1 hour to complete). This recall remedy is estimated to be available beginning on August 12, 2022. In Type text here the meantime, Kentucky Trailer

was not able to isolate the defect to a particular year or production run. As such, the recall population includes all of the approximately 2944 truck body units manufactured by Kentucky Trailer

truck body units, which serves as the housing for each unit’s walk board while in transit, can become loose due to vibrations, damage and other forces loosening the 28 nut/bolt fasteners that attach the walk board rack to the underbelly of the truck body unit. If enough fasteners become loose or separated, there may be enough movement in the walk board rack to allow the walk board to fall off of the unit. (KT) personnel were notified by a customer that a walk board rack mounted to its truck body was missing 4 nuts/bolts which caused it to became loose and led to the walk board Part 573 Safety Recall Report 22V-419 Page 3 The information contained in this report was submitted pursuant to 49 CFR §573 itself falling from the truck body unit. The incident did not result in any personal injury or property damage, other than damage to the walk board itself. This was the first report of such an incident since KT began manufacturing truck bodies in 2014. It was initially suspected that the nuts/bolts had been intentionally removed in an attempt by someone to steal the walk board. However, KT then developed a plan to locate and test additional nuts/bolts on similar units that were operating in the field. KT sales/marketing team members reached out to certain customers in an effort to locate similar units that could be made available for inspection. Given it is “moving season”, units that could be made available for inspection proved difficult to locate; however, on June 2, 2022 KT engineering and quality personnel inspected 2 truck body units manufactured in 2016 and 1 manufactured in 2017. The applicable nuts/bolts on 1 of the 2016 units and on the 2017 unit were all secure. However, certain nuts/bolts on the other 2016 unit were either loose or missing (KT replaced all nuts/bolts on this unit the same day). On June 3, 2022, KT engineering personnel inspected 4 KT truck body units (all manufactured in the 2020/2021 time frame) and confirmed all applicable nuts/bolts on the units were secure. On June 6, 2022, KT engineering personnel inspected 2 KT truck body units manufactured in 2018 and determined that certain nuts/bolts on the units were either loose or missing. On June 7, 2022, KT convened a meeting of certain management personnel and its board of directors and made a good faith determination that certain KT truck body units contain a safety related defect and that KT should proceed to notify NHTSA and initiate a recall. intends to remedy the subject defect by removing all nut/ bolt fasteners installed on the walk board rack assembly (~28) and replace them with a new washer, bolt and locking nut (Stover brand) that is torqued to the appropriate specification. Kentucky Trailer
intends to undertake this service work at any of its service facilities at no charge to customers. Kentucky Trailer
also intends to make available for shipment to customers (or their designees), packages containing the required washers, nuts and bolts along with replacement instructions (including detailed diagrams and specifications) and to reimburse customers for the reasonable cost of the completed service work. How Remedy Component Differs from Recalled Component : The nuts previously utilized in Kentucky Trailer
’s production process to mount the walk board rack to the truck body are not “locking” nuts. Thus, Kentucky Trailer
intends to utilize “locking” nuts (Stover brand) in connection with its defect remedy program and also in its production process going forward.
