Pedal assemblies applied to heavy duty vehicles cannot exhibit the failure mode due physical constraints in the heavy duty vehicle installation. Production Dates : FEB 01, 2021 – FEB 01, 2023 Description of Defect : Description of the Defect : The pedal plunger that connects the brake pedal to the brake valve may become disconnected or jammed if the pedal is fully applied then released extremely rapidly (slip-foot brake release). The slip-foot brake release may allow the pedal to overshoot the travel limit stop disconnecting the plunger from the foot brake valve. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : The disconnected or jammed plunger does not transmit force or motion from the brake pedal to the brake valve. The service brakes cannot be applied increasing the likelihood of a crash. Description of the Cause : Adjustment of the gap between the travel limit stop and the brake pedal may be excessive allowing the plunger to become disconnected or jammed. The disconnection or jammed pedal plunger is always preceded by a “slip-foot” brake release which is a very rare event. Bendix was informed that the point of discovery at body builder when moving truck into their shop and driver’s foot slipped off the pedal, dislocating the brake pedal plunger. Body builder called dealer and dealer investigated and was able to recreate on four more trucks in dealer inventory. Jan 23, 2023 – Bendix began quality containment of parts produced at Bendix. Jan 23, 2023 – Jan 27, 2023 Bendix engineering worked to reproduce the issue. Parameters and material characteristics were analyzed. Use-cases that lead to pedal plunger disengagement were evaluated. Jan 27 2023 – Bendix Product Integrity Committee opened a Risk Assessment to clarify the potential vehicle safety risk. Feb 2, 2023 – Bendix Product Integrity Committee determined that a safety defect exists. Description of Remedy : Description of Remedy Program : The remedy program has not been defined. The excessive gap between the pedal travel limit and pedal was eliminated and the pedal plunger cannot become dislocated or jammed. Recall Schedule : Description of Recall Schedule : Only one vehicle manufacturer purchases the pedal assembly and they will be notified in writing on or before April 1, 2023. There are no direct sales to vehicle owners or aftermarket customers.
