Summary
Cummins
, Inc. (Cummins
) is recalling certain L9
part number D0K3003BX03 and ISX12 part number D563025BX03 diesel engines. The sealing washer may not seat correctly in the pilot bore holes, allowing the high pressure fuel rail assembly to leak.
Remedy
Dealers will inspect the rail threads and fuel lines, and replace the rail as necessary, free of charge. Owner letters were mailed July 30, 2021. Owners may contact Cummins

Issue
Symptom:
External Fuel Leak from the pressure relief valve or the high-pressure fuel fitting on the fuel rail.
Root Cause:
The rail end sealing bores in the high-pressure fuel rail may have undersized pilot bores for the sealing washer, thus preventing the washer from properly seating, potentially resulting in inadequate load for the joint to remain properly sealed in service.
Verification
- Remove the pressure relief valve and the high-pressure fuel fitting from the fuel rail.

- Remove and discard the sealing washers. The procedure below outlines how to remove the sealing washers.
CAUTION
Special care should be taken to avoid damage to threads during removal process.
- A special tool must be created for the removal of the sealing washers by grinding a 45 degree angle on the short leg of a 1/8-inch or 3/16-inch Allen wrench, so that the wrench is no longer than 13 mm [ ยฝ in ] long (measured from the outside of the long leg.)
- This tool acts as a mini
heel bar to pry out the sealing washer without damaging the back of the hole.
- Use the modified Allen wrench to pry out the old sealing washer from the threaded hole in the fuel rail. Considerable force is required to remove the sealing washer.

- Visually inspect the thread surfaces of the fuel rail, fuel pressure relief valve and the high-pressure fuel fitting for damage. Review Figures 3 and 4 below for guidance.
- Do not reuse components if the threads or sealing surfaces are damaged.

Resolution
- A new seal installer tool, Part Number 5573487, is now available for this repair.

- The new seal installer tool must be available for all repairs requiring the installation of pressure relief valve or the high pressure fuel fitting on the fuel rail. See Service Instructions section below.
CAUTION
Do not perform the repair if the seal installer tool is not available or is damaged as damage to components can result.
Service Instructions
- If the threads or sealing surfaces are damaged, replace the fuel rail assembly. See corresponding Service Manual. Reference Procedure 006-060, 006-061 in Section 6 and Technical Service Bulletin, New Fuel Rail, High Pressure Injector Supply Lines, Fuel Supply Line, and Fuel Supply Line Braces, TSB210055 for directions.
- If the threads or sealing surfaces are not damaged, install the pressure relief valve and the end fitting with the following procedure:
1. Inspect the seal installer tool for damage on the threads or sealing surfaces. Do not use the seal installer tool if the threads or sealing surfaces are damaged.

2. Install a new sealing washer on the pressure relief valve bore in the fuel rail. Use assembly lubricant, Part Number 3163087, or equivalent, on the threads and the face of the seal installer tool. Use seal installer tool, Part Number 5573487, to seat the sealing washer fully in the fuel rail bore.
3. Torque the seal installer tool.
Torque Value: 81 nโขm [ 60 ft-lb ]

4. Remove the seal installer tool after the sealing washer has been installed. Verify that the sealing washer remains seated inside the fuel rail after the seal installer tool is removed.

5. Inspect the seal installer tool for damage on the threads or sealing surfaces. Do not use the seal installer tool if the threads or sealing surfaces are damaged.
6. Install a new sealing washer on the high-pressure fuel fitting bore in the fuel rail. Use assembly lubricant, Part Number 3163087, or equivalent, on the threads and the face of the seal installer tool. Use seal installer tool, Part Number 5573487, to seat the sealing washer fully in the fuel rail bore.
7. Torque the seal installer tool.
Torque Value: 81 nโขm [ 60 ft-lb ]
8. Remove the seal installer tool after the sealing washer has been installed. Verify that the sealing washer remains seated inside the fuel rail after the seal installer tool is removed.
9. If either of the sealing washers do not remain seated inside the fuel rail after the seal installer tool has been removed, use of seal installer tool is not required. Install washer in bore.
10. Install the pressure relief valve and the high-pressure fuel fitting. Use assembly lubricant, Part Number 3163087, or equivalent, on the threads. Tighten the pressure relief valve and the high pressure fuel fitting with the torque plus angle method.
Torque Value: 27 nโขm [ 239 in-lb ] Rotate 90 degrees.

Description
This Safety Campaign is being issued to address a defect which relates to safety that exists in certain Cummins
L9
and ISX12 engines manufactured between January 2, 2017 and October 16, 2020.
The fuel rail assembly may develop leaks, which may result in an undetected prolonged diesel fuel spray. The operator may see or smell diesel fuel and, in some cases, the check engine lamp may illuminate. A leak involving spraying/misting fuel in the presence of an ignition source may increase the risk of fire.
Note : This field action provides retroactive coverage for repairs made in anticipation of and prior to the release of this publication.
Action
In order to qualify for repair under this field action, an engine:
1. will be covered Regardless of coverage status, and
2. must show as OPEN on QuickServeยฎ Online for this field action.
Note : The ESN list is attached for reference.
After verifying that the engine meets the above requirements, perform the following actions:
1. Remove the fuel pressure relief valve and the end fitting from the fuel rail. Refer to Procedure 006-061 in the appropriate service manual for further detail.
2. Remove and discard the sealing washers using the appropriate tool. Refer to TSB210113 and 006-061 in the appropriate service manual.
Note : A special tool must be used when removing the sealing washer to prevent end thread damage upon removal of the washers. Please refer to TSB210113 and 006-061 in the appropriate service manual for instructions to create the sealing washer removal tool. If the repair location is unable to create this tool, please reach out to the local Cummins
Distributor branch for assistance.
3. Visually inspect the end thread surfaces of the fuel rail for damage. Refer to TSB210113 for visual examples.
4. Based on inspection results, follow Repair Path A or B as outlined below.
Repair Path A: If no damage to fuel rail end threads exists;
4. A.1. Complete installation of new washers. Refer to TSB210113 and 006-061 in the appropriate service manual.
Note : A new Seal Installer Tool, Part Number 5573487, has been released for use in this repair and MUST be used to install the washers. This tool is not a consumable and can be used to complete multiple repairs. Each Repair Location will be allowed to order up to three of the Seal Installation Tools, PN 5573487, to complete the repairs described by this Campaign. Orders of more then 3 of 5573487 will be cancelled unless ordered per the instructions in PPC# 6043.
4. A.2. Place a paint dot on the fuel rail to indicate that the washer installation has been completed.
4. A.3. File one claim for only the applicable parts, labor, access, and travel or towing listed in this field action.
Repair Path B: If the fuel rail end threads are damaged, see TRP2456 for rail replacement parts and repair instructions.
4. B.1. After completing TRP2456, file two claims:

Purpose
The purpose of this memo is to notify repair locations that Cummins
Inc. is initiating interim end user notification mailings regarding a safety defect on certain L9
and ISX12 engines.
Discussion
Cummins
Inc. has decided that a defect which relates to safety exists in certain L9
and ISX12 engines that were manufactured between January 2, 2017 and October 16, 2020 and installed in bus, school bus, emergency vehicle, recreational vehicle, generator set and marine applications. The fuel rail assembly may develop leaks, which may result in an undetected prolonged diesel fuel spray. A leak in the presence of an ignition source may increase the risk of fire.
Cummins
has notified affected Original Equipment Manufacturers to which these engines have been shipped and has filed a Defect Notification with the U.S. National Highway Traffic Safety Administration for applicable engines. NHTSA has assigned this recall number 21E-032.
The Safety Campaigns are expected to be published on QuickServe Online in July, 2021 when the parts, special tooling and repair instructions are expected to be available. Safety Campaign
Number C2423 will apply to on-highway units in the United States and Canada and Safety
Campaign Number C2425 will apply to all other units. Cummins
will contact end-user by mail when the Campaigns are published.
Action
Until the Safety Campaigns are available, for affected units that develop a leak, follow the repair identified in TSB210113. For units that are not leaking, distributors and dealers must wait until the appropriate Safety Campaigns are published on QSOL to effect any repairs.
Cummins
expects to begin notifying owners on June 18, 2021 with interim letters informing them that the safety defect exists and that they will receive another notice when the repair is available. Cummins
is advising customers to periodically check the fuel rail for signs of leaking and seek repairs if leaks are identified. If a leak occurs, the operator may see or smell diesel fuel and, in some cases, the check engine lamp may illuminate. A second notification will be sent to end users to inform them when the final remedy is available and provide instructions to schedule the recall for their engine. The second notifications are expected to mail during July 2021.

- A. Remove the pressure relief valve from the fuel rail. Refer to Procedure 006-061 in the appropriate service manual for further detail.

- B. Remove the #1 injector supply line from the fuel rail, as well as the high-pressure fuel fitting, from the fuel rail. Refer to Procedure 006-051 in the appropriate service manual for the injector supply line and TSB210113 for additional information on the high-pressure fuel fitting.
- NOTE: Only the #1 injector supply line needs to be removed for this repair to access the sealing washer on the left side of the fuel rail. Only Claim 1 count each of the appropriate SRT for Injector Supply Line Remove and Install Each – Additional.
2. Remove and discard the sealing washers using the appropriate tool. Refer to TSB210113.
NOTE: A special tool must be used when removing the sealing washer to prevent end thread damage upon removal of the washers. Please refer to TSB210113 for instructions to create the sealing washer removal tool. If the repair location is unable to create this tool, please reach out to the local Cummins
Distributor branch for assistance.
NOTE: There are 2 sealing washers that need to be installed, one located on each end of the rail.
3. Visually inspect the end thread surfaces of the fuel rail for damage. Refer to TSB210113 for visual examples.
4. Based on inspection results, follow Repair Path A or B as outlined below.

We are writing to inform you that Cummins
Inc. has determined that a safety-related defect exists in certain L9
and ISX12 engines that were manufactured between January 2, 2017 and October 16, 2020. Cummins
has initiated a recall of the subject engines. Our records indicate that some of these engines have been shipped to you.
A copy of Cummins
โ initial Part 573 Defect Notification report is enclosed. This notification was submitted to the National Highway Traffic Safety Administration (NHTSA) on April 20, 2021. As more fully described in the Defect Notification, fuel rail assembly may develop leaks, which may result in an undetected prolonged diesel fuel spray. A leak involving spraying/misting fuel in the presence of an ignition source may increase the risk of fire.
Enclosed is a list of the affected systems that our records indicate were shipped to your plants. Reimbursement Program โ NHTSA Recall No. 21E-032
Reimbursement Program: Our program for reimbursing U.S. customers who, prior to this recall, incurred costs to have the defect described in the recall notice addressed is as follows (subject to the limitations and qualifications below):
We will reimburse the customer the lesser of the amount the customer paid to have the remedy performed or the customerโs cost of remedy parts/components (at the Cummins
list price for related, authorized parts/components), labor at local rates, and associated costs such as taxes and disposal fees.
Time Limitation: To be eligible for reimbursement, the remedy must have occurred on or after April 20, 2020 (i.e., one year prior to Cummins
โ April 20, 2021 notification to NHTSA) and no later than ten (10) calendar days after Cummins
completes its notification campaign to owners.
Exclusions: Cummins
โ program does not provide reimbursement for the following:
- Costs incurred while the original or extended warranty was in effect, unless our authorized dealer or representative denied warranty coverage or the warranty repair did not remedy the problem involved in the recall;
- Repair that did not address the defect involved in this recall;
- Repair that was not reasonably necessary to correct the defect involved in the recall;
- Repair of equipment that was first purchased more than 10 years before the recall notice; and,
- Claims submitted without proper documentation, as specified below. You will be given an opportunity to resubmit the claim with the complete information.
Required Claim Documentation: To process your claim, you must submit the following information to Cummins
: your name and address; a description of the equipment; the recall number; the name of the owner of the equipment when the work was performed; a copy of the receipt for the work performed; documentation indicating that the claimant owned the recalled equipment (such as an invoice or receipt indicating the purchase of the recalled equipment); and, if the work was performed when the equipment could have been remedied at no charge under warranty, documentation indicating that our authorized dealer or facility either refused or failed to remedy the recall problem.
Timing of Response to a Claim for Reimbursement: A response to a claim for reimbursement will be sent within 60 days of receipt of the claim. If the claim is denied due to a failure to provide complete documentation, the claimant may resubmit the claim with the complete documentation. Resubmitted claims must be received by Cummins
within 60 days of the date of the denial.

Chronology :
February 10, 2021 โ A field service technician reported to Cummins that eight buses belonging to a single customer had been repaired since December 2020 for leaks at sealing washers in the fuel rail.
February 11- March 1, 2021 โ Cummins investigated the cause of the leaks with the supplier of the rail and began an investigation of field warranty claims. Cummins conducted an initial Product Safety Hazard Analysis.
March 3 โ April 12, 2021 โ Cummins escalated the issue through its Product Safety Defect Board process, gathered additional data and revised the Product Safety Hazard Analysis. Cummins issued a Technical Service Bulletin (TSB210055) on March 4, 2021 to allow the field to replace rails that have washer leaks with a different rail and their associated fuel lines. (TSB210055 was submitted to NHTSA on April 7, 2021 in accordance with 49 CFR Part 579.5.)
April 13, 2021 โ Based upon the results of the investigation, Cumminsโ Product Defect Safety Board decided to conduct a safety campaign on engines installed in buses, school buses, emergency vehicle and recreational vehicle applications and certain other off-road applications, including generators.
May 19, 2021 – Cummins determined that 378 L9 engines that were mounted by Cummins Power Systems onto trailers for use in mobile generator set applications should be added to the recall population.
To date, there have been no reports of accidents, fires or injuries related to this condition.
